Description

Responsible for providing case management services. Work schedule will reflect meeting the requirements under the essential job functions and will be overseen by the Director of Supervised Independent Living. All employees of Boysville, Inc. receive training in the Quality Improvement (QI) activities of the company and are required to actively participate in QI meetings, tasks, projects and/or committees, as needed.

Essential Job Functions:

• Provide case management services for Boysville residents
• Complete social histories involving initial intake interview with child and support system(s) for consideration of long-term residency.
• Complete intake documentation and build resident file
• Complete monthly Chronologicals for all SIL residents
• Maintain a complete computer and paper file on each child in case load
• Attend regular ARDs and court hearings
• Complete regular six-month Transitional Living Service plans
• Complete Quarterly Reviews
• Oversee arrangements for special needs of children during holidays or other special events.
• Participate in the planning and execution of SIL Camp Independence, Prom, Senior pictures, monthly Skills groups, etc
• Participate in Administrator on Duty rotation
• Attend Interdisciplinary Team Meetings and Assessment Team Meetings weekly
• Attend Administrative program Meetings
• Attend SIL Team Meetings
• Complete monthly budgets, contact logs and Chronologicals for all SIL residents
• Assist in some transportation needs for residents
• Inspect cottage cleanliness and compliance

Qualifications

• Must be 22 years or age or older
• Must meet the following education and experience requirements
  • Bachelor’s Degree in Social Work or related Human Services field and 1 year of documented experience
    • (Licensed Bachelor Social Worker and 1 to 2 years experience in a similar position, preferred), or
• Must possess excellent communication, organization, time management and written skills.
• Must be computer literate in windows applications.
• Must exhibit behavior that demonstrates cultural diversity and sensitivity (See Corporate Policy #2103).

• Ability to read, write and converse in English
• Ability to communicate with staff, residents, families, vendors, support agencies and others
• Ability to handle stress
• Bending, lifting, grasping, fine hand/eye coordination, pushing/pulling, prolonged sitting/standing
• Ability to respond to telephones and other auditory stimulation.
• Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements).
