POSITION SUMMARY: The Clinician will conduct needs assessments, work with youth and family in developing plan of care and provide short-term, solution focused interventions. Responsible for synthesizing all information gathered into the assessment process, and conduct individual and family counseling.

• Coordinates and ensures implementation of all clinical services as indicated by youth’s individual plan of care.
• Provides assessments, individual and family counseling.
• Complete training in Cognitive Behavioral Therapy Intervention for Trauma in Schools (CBITS) and provide CBITS intervention to clients.
• Assists Program Manager in arranging admissions and discharges of youths.
• Collaborates with mentors regarding implementation of service plans.
• Completes assessments, progress notes, service plans, and discharge summaries and other paperwork as assigned.
• Participates in program trainings both internal and external as required.
• Coordinates and attends all case conferences, family meetings and providers meetings.
• Complete comprehensive Bio-Psychosocial assessment for all youth admitted within 30 days of admission.
• Facilitates the completion of the evaluation for youth removed for the first time
• Follows all state, federal, and agency regulations regarding documentation and billing.
• Provides discharge planning, including referrals to community-based services.
• Collaborates with DCF, schools, legal system, and other service providers as indicated.
• Keeps Program Manager apprised of relevant case dynamics and risk factors.
• Provides crisis intervention as needed.
• Arranges flexible work schedule to provide needed services to youths.
• Provides after hours on call.
• Master’s degree in social work or the equivalent, license eligible
• Experience in delivery of individual and family counseling

PURPOSE/FUNCTION OF THE PROGRAM:

The Kraft House is a 12-bed inpatient psychiatric residential treatment program for adolescent boys, including those boys with substance use disorder who have experienced emotional, physical, or sexual abuse. This program is a step down from inpatient hospitalization or a diversionary placement from a more restrictive environment. The program is designed to help adolescent boys return to home to their community or a long- term group- home setting.
