Hourly Rate: Starting at $18.00/hour comprehensive benefits
Pay Range: $18.00 to $20.00/hour
Hours: 30 hours/week, occasional evening and weekend hours
Location: ARCHES
Job Summary: Connect families in the community with necessary health and basic needs resources. Assist with the development of ARCHES programs and integration of services with Club and school-based sites.
Essential Duties & Responsibilities
Conduct intakes with clients and families
Complete weekly phone calls to clients on waiting list as needed and answer phone calls to provide customer service
Call and schedule clients for intakes and appointments according to program procedures and complete follow up phone calls for reminders or missed appointments
Provide referrals and community resources to clients and families
Document all services rendered including phone contact, completed services and record in database
Attend staff/volunteer meetings and trainings
Ensure a productive work environment by working as a team member, contributing to team meetings and providing feedback to support decision making; continuously manage own time and resources
Continuously develop own knowledge and skills in each of the various Community Health Services programs in order to diversify skills
Be resourceful and possess a knowledge of community based mental health and family services
Effectively use Microsoft Office applications, electronic scheduling and database software
Build partnerships with parents, families and community partners
Maintain close, daily contact with staff, members and supervisors to receive and provide information, discuss issues, explain or interpret guidelines and instructions
Maintain contact as needed with external community groups, schools, members, BGCGG partners and others to promote and ensure information and referral services for the Community Health Services Programs are successful
Ensure the ARCHES office meets health and safety standards and ensures the safety of all clients
Work with diverse populations in a culturally competent manner
Maintain strong organizational skills with attention to detail, follow-up and manage demanding caseload
Maintain strong communication skills; both written and verbal, and provide feedback regularly
Maintain knowledge of HIPAA and confidentiality regulations; follow organizational procedures
Act as English/Spanish or English/Vietnamese translator when interpretation is needed for services and activities related to case management and facilitating Medi-Cal applications
Medi-Cal Related Responsibilities
Attend trainings regarding the Medi-Cal screening process, including updates regarding changing requirements
Assist families in applying for Medi-Cal, if applicable, and discusses follow-up health care
Act as a patient advocate for clients requiring assistance with healthcare/provider issues for the purpose of facilitating access to Medi-Cal health insurance coverage
Actively encourage parents regarding enrolling uninsured children in health insurance
Advocate for families enrolled in Medi-Cal needing health care services by assisting them in utilizing Medi-Cal covered health care services, and providing follow-up with families as needed
Encourage health prevention and awareness
Arrange transportation for clients to access health services
Additional Duties & Responsibilities
Assist in planning community outreach events, develop marketing materials, and attendcommunity events
Assist in collecting, entering and compiling data relating to clients served and services provided
Assist in screening, selecting and enrolling clients, and follow up with phone contact as needed
Maintain skills in project management; skills in providing training; networking skills and ability to establish collaborative relationships
Qualifications
Qualifications
Must be bilingual in Spanish or Vietnamese and at least 18 years of age
AA degree and 2 years relevant experience OR 1 year case management experience
Experience working with families and youth
CA driver license, reliable transportation, safe driving record, 2 years of driving experience and automobile insurance required
Position may require standing/sitting for extended periods of time, dexterity of hands and fingers to operate office equipment, reaching overhead, above the shoulders and horizontally, bending at the waist; seeing to review and analyze reports, documents or other reports; hearing and speaking to communicate and provide information to others; speaking to be heard before groups of people; position may require to lift, carry, pull or otherwise move objects between 10-20 pounds
