BOSSGO, INC. is Hiring a Resident Care Management Director Near Vancouver, WA

Who we are looking for:
We are seeking a highly motivated and experienced Resident Director to join our team. You need to have an Active RN license in the state of WA. A minimum of 3 years as a nurse working with long term patients. This is a leadership role that requires strong management skills and a passion for providing leadership. As a Resident Director, you will be responsible for overseeing the daily operations and care of our residents.

Job Title: RCM (Resident Care Manager)

Job Summary: RCM will ensure resident needs are meant and that caregivers are providing exceptional care to our residents. Will work hard to ensure all employees are properly trained and team morale is high. Your primary responsibility is to ensure the safety, comfort, and well-being of patients by administering medications, performing treatments, and monitoring their condition of residents for Better Options for Elder Care/Rite Choice Ventilator Specialty within compliance of the WAC.

Manage Resident

  • Manage Resident Care
  • Conduct regular assessments of resident needs and conditions, coordinating with healthcare providers to adjust care plans as necessary
  • Assess status of each resident and identify any specific patient needs required for care
  • Delegate said items or perform those actions, as appropriate
  • Follow up on any delegated actions
  • Lead any admissions or discharge activities
  • Process orders for medications and equipment
  • Develop care plans and assessments before/on admission date
  • Coordinate with other departments, such as dietary, therapy, and social services, to ensure holistic care and meet the diverse needs of residents
  • Make daily and night shift observations of caregiver performance to
  • Note need for immediate coaching of caregiver
  • Note trend in deficient practices for future training
  • Perform and document skill checks in caregiver records
  • Ensure adherence to policies and procedures
  • Identify possible amendments or additions to policies and procedures
  • Provide leadership and supervision to the nursing staff, including registered nurses, licensed practical nurses, and certified nursing assistants, ensuring adherence to facility policies, procedures, and best practices.
  • Develop training for admissions and discharge activities for other caregivers
  • Process orders for medications and equipment
  • Develop care plans and assessments before/on admission date
  • Ensure ADL’s, medications/treatments, and the daily care plan are all reflected in the EMAR system.
  • Ensure communication of daily care plan for each resident to subsequent shift
  • Coach caregivers in communication best practices of daily care plan for each resident at change of shift
  • Work on the floor weekly a one 12-hour shift and as coverage or when needed for emergency
  • Manage Policies and Procedure Documentation
  • Serve as a liaison between residents, families, and healthcare providers, addressing concerns, communicating updates, and facilitating care coordination
  • Promote a culture of safety, infection control, and resident dignity and rights within the facility, addressing any violations or concerns promptly and effectively
  • Regularly review policies and procedures to
  • Update for compliance with State guidelines
  • Update for continuous improvement based on observations and feedback
  • Develop new policies/procedures based on State input or observations
  • Ensure caregiver understanding and competency with policies and procedures through training
  • Ensure timely and complete recordkeeping
  • Resident charts
  • Yearly care plans reviewed and updated
  • All nurse delegation forms in chronological order by date
  • Secure medication updated and signed by physician every six months
  • Dr’s Care Plan updated and signed by physician every six months
  • Keep state required documents signed
  • Ensure compliance with state and federal regulations and accreditation standards related to resident care, safety, and documentation
  • Electronic records
  • Ensure all med passes are marked as completed
  • Patient information is in chart
  • Medication is correct in system compared to medication order
  • Audit medication carts for expired and discontinued medications monthly; may be delegated with appropriate follow up to ensure compliance
  • Through ongoing education and professional self-development of activities, you can stay current with nursing best practices, regulatory requirements, and industry trends

Requirements:
- Bachelor's degree in Science of Nursing
- Previous experience in dementia care, memory care, or assisted living strongly preferred
- Strong knowledge of care plans, home care practices, and nursing home regulations
- Excellent leadership skills with the ability to motivate and inspire a team
- Compassionate and patient demeanor with a genuine desire to help others
- Strong organizational skills with the ability to multitask effectively
- Excellent communication and interpersonal skills

-Completion of state required certificates (mental health, dementia) within 120 days of hire
