Borinquen Medical Centers is based in Miami, FL and is seeking to hire a full-time Clinical Care Coordination Specialist (CCCS). The CCCS will support the organization’s aim to strive for clinical and service excellence. Maintains collaborative team relationships with peers and colleagues to effectively contribute to the achievement of goals, and to help foster a positive work environment. The Clinical Care Coordination Specialist is primarily responsible for patient experience and promoting clinical excellence. He/she addresses patient feedback and incidents. In doing so, he/she runs reports and conducts trend analyses to guide service improvement initiatives such as training, new/updated processes, etc. He/she works on improving and standardizing clinical care by evaluating the outcome of clinical support staff driven clinical/quality measures. In doing so, he/she runs reports and conducts trend analyses to identify areas of focus, evaluate workflows, identify knowledge gaps, and implement improvement initiatives. He/she works in collaboration with providers, clinical managers, and the rest of the clinical team to optimize patient experience and outcomes.

DUTIES AND RESPONSIBILITIES

  • Routinely send Patient Experience Surveys to all patients.
  • Routinely send Ryan White Experience Surveys to all case management clients.
  • Provide and present monthly patient/client experience survey results for each clinical department during monthly meetings.
  • Perform monthly site walk-through audits to support the ongoing implementation of AAAHC standards.
  • Receive and respond to patient feedback and incidents in a timely manner and document the resolution/follow-up action appropriately.
  • Round on patients routinely.
  • Provide oversight on the follow-up and documentation of after-hours calls.
  • Provide oversight on ensuring emergency boxes are up to date.
  • Provide oversight on the follow-up and documentation of emergency medicine referrals.
  • Monitor tasks in the clinical inbox for timely action.
  • Provide in-services to promote the improvement of patient experience and clinical outcomes.
  • Provide oversight on the follow-up abnormal screening results (i.e. cervical cancer, mammogram).
  • Track and document employee training as it relates to risk management requirements.
  • Attend clinical department monthly meetings in-person.
  • Design and initiate special projects under the direction of the Director of Clinical Services & Risk Management.
  • Maintains awareness of compliance and regulatory/accreditation requirements and deficiencies and notifies the Director of Clinical Services & Risk Management of any actual or potential risk situations.
  • Makes decisions applying critical thinking as a process towards resolution.
  • Maintains confidentiality of all files and allows access only to persons appropriately involved in the resolution of the concern.
  • Demonstrates excellent conflict management skills to achieve issue resolution.
  • Maintains ownership for communication of resolution or response to all parties involved.
  • Demonstrates proficiency in application of service recovery consistent with organizational policy and best practice.
  • Promote and apply AIDET in appropriate situations.
  • Maintains comprehensive, accurate, and timely documentation of all feedback received from patients, families, or guests.
  • Maintains knowledge of BHCC services and resources available to patients.
  • Other duties as assigned by immediate supervisor as required.
REQUIREMENTS
  • Medical Assistant, LPN, or RN license or at least 5 years of health care experience. Additional related experience may substitute for required education on the same basis. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job.
