Job Title: SAMHSA MAT Peer Navigator/ Community Outreach Worker

Job Summary: The Peer Navigator/Community Outreach Worker will assist the Program Director with organizational outreach/engagement/screening/recruitment of new participants. The CHW will function as a role model, using a person-centered approach to engage and serve as an advocate providing peer support and resources for clients served.

Primary Duties & Responsibilities include, but are not limited to:

• Conducts field, and street outreach to locate and engage individuals who have opioid use disorder to Suboxone service.

• Conduct intake assessments including review of substance use history, current drug use practices, medical waste disposal practices, and providing treatment planning and education to participants within the agency and on the mobile unit.

• Facilitate access to onsite HRC services, provide risk reduction education, support follow-ups to ensure retention in care, and schedules and verify support service appointments for clients.

• Participate as part of the BOOM!Health’s mobile van outreach team to locations where high-risk persons may be found.

• Maintain documentation on number of opioid users contacted through mobile van outreach and number of outreach educational presentations with attendance made at SUD-related agencies.

• Work with management to identify new networking opportunities to meet monthly quotas.

• Escort participants, as needed, to various appointments via Public Transportation.

Qualifications/Criteria:

• Minimum of one year of experience providing harm reduction counseling, referrals, prevention education, safer substance user strategies, and education.

• Demonstrate a positive attitude in an environment of constant change that will support the needs of a person entering treatment, in crisis, or in need of an escort/additional support will require. • Knowledge of infectious disease transmission and prevention practices for injectors (HIV, Hepatitis C, Opiate Overdose Prevention and Response) health education, outreach strategies, harm reduction theory, and practice.

• Must be able to effectively engage individuals in the community and provide risk reduction education and information.

• Must be knowledgeable about Buprenorphine and other medications used to treat opioid use disorder or the ability to attend training.

• Good communication skills oral, written, and interpersonal skills.

• Some computer skills are required including experience using MS Word.

• Bilingual (English/Spanish) is preferred.

• Available to work a flexible schedule, including afternoons, evening, and weekend rotations.

• Physically able to climb stairs, move and lift boxes and supply packs weighing up to 20 pounds.

Education: High school or equivalent (Preferred)
