Position Summary
RN Liaison coordinates referrals of patients from to Hospice from the inpatient or outpatient setting. Ensures patient eligibility for hospice and assists with discharge planning and scheduling of admissions upon discharge to home, facility or other residence. Builds referral relationships and engages in awareness and marketing events.
Essential Functions
Serves as hospice expert and resource with hospitals, facilities, and other referral sources. Builds relationships with hospital care teams, palliative care, discharge planners, community representatives, senior centers, long term care facilities, skilled facilities, physician practices, etc.
Attends hospital rounds, meetings, case conferences, family meetings, etc. to identify patients referred/eligible for hospice care.
Provides resources and education in the hospital, facility and home setting as needed regarding hospice eligibility, services, level of care and regulatory clarification. Collaborates with business development to increase awareness and access to hospice care.
Responds to referrals within 3 hours. Follows pending referrals daily and communicates status with the Clinical Coordinator and admitting team.
Schedules meeting with patient/family member to explain hospice services, discuss care options, and coordinate discharge plan.
Identifies patient/family’s physical, psychosocial and spiritual needs and preferences that may influence or impact patient’s care.
Collaborates with palliative care, providers, care team, discharge planners or others involved in the patient’s care to ensure continuity of care and goals of care for each patient.
Determines eligibility and appropriateness for hospice care based on state and federal guidelines and in collaboration with the hospitalist, patients attending physician and hospice medical director.
Orders durable medical equipment, supplies and medications in preparing for hospice admission.
Functions as a member of the hospice team, answering and making calls, performing admissions and visits as assigned. Serves as mentor or preceptor for new staff as assigned.
Job Requirements
Graduate of an accredited school of nursing. BSN Degree preferred.
Current licensure to the practice as a Registered Nurse in the Commonwealth of Virginia
At least two years of experience in hospice care, case management, sales and marketing experience preferred.
Drivers License in good standing and acceptable record
Other Knowledge, Skills and Abilities
Customer service oriented with strong verbal, oral and written communication skills. Must relate well others and possess strong interpersonal skills. Must be proficient in Microsoft office applications, email, and internet searches. Must have effective time management and organizational skills
