MDS COORDINATOR
The Steven and Alexandra Cohen Pediatric Long-Term Care Pavilion at Blythedale Children’s Hospital is seeking a MDS Coordinator for our 24-bed facility. This is an exciting part time opportunity for an experienced MDS Coordinator to make an impact on the delivery of medical care to children and their families who are challenged by complex medical issues. The successful candidate will have an established reputation for leadership, teamwork and partnering with residents and families.
As a the MDS Coordinator the successful candidate will oversee accurate and thorough completion of the Minimum Data Set (MDS), care area assessments (CAAs), comprehensive care plans (CCP) and interim care plans, in accordance with federal and state regulations and guidelines that govern the process. Working in collaboration with the interdisciplinary team (IDT) participate in regular care planning meetings. Ensure all new admissions and re-admissions are evaluated within 24 to 72 hours for Medicaid eligibility and/or significant changes in medical status and report such changes to the Director of Nursing. Monitors the Case Mix Index scores and CASPER report looking for potential risks, while ensuring all regulatory reporting and guidelines are followed.
Requirements: Bachelor of Science Degree in Nursing required. Licensed, or eligible for licensure by the New York State Department of Education as a registered professional nurse. A minimum of 1 years MDS experience or 3 years Long Term Care experience, Pediatric RN experience required. BLS certified, PALS encouraged. Must exhibit fine motor coordination in the manipulation of biomedical equipment (e.g., thermometers, scissors, etc.).
