PACE is an innovative program designed to provide and integrate all acute, primary, preventive and supportive health services to high-risk individuals living in the community. Our goal is to keep our members living safely in their homes for as long as possible. We provide services in the community and at our PACE center which is open 8 AM to 5 PM Monday – Friday.
In addition to managing the day-to-day functions of the PACE clinic, the Clinic Nurse anticipates and supports the needs of our members through ongoing assessment, planning, coordination, real-time intervention and follow-up.
This position requires strong critical thinking and problem solving skills with a focus on preventing unnecessary emergency, hospital and skilled nursing utilization. The Clinic Nurse works closely with our physician and other team members to effectively monitor, identify and address any potential risks to the health and wellness of those we serve. This position will also be responsible for providing the highest level of customer service and compassion to our members and their families.
The Clinic Nurse is an integral part of our interdisciplinary team. Within this team, the Clinic Nurse will participate through open communication, active listening, collaboration and multi-disciplinary planning. Successful candidates will exhibit strong clinical and leadership skills.
