Bluestem Health is Hiring an Outreach Worker Bilingual Spanish/English Near Lincoln, NE

Description

The Outreach Worker is responsible for coordinating services and educating patients on Bluestem Health’s sliding fee scale and Medication Assistance Program (MAP). This position will coordinate follow-up phone calls regarding expiring sliding fee scales, MAP, and be able to answer any other questions regarding the MAP or sliding fee scale programs. Other duties and responsibilities include increasing access to care through application and enrollment assistance for people who may be eligible for marketplace insurance. 

1. Respond to incoming requests for assistance regarding the application and enrollment process

2. Provide information in a fair, accurate, and impartial manner

3. Remain current with eligibility requirements

4. Work cooperatively with the providers and personnel to carry out goals and objectives of 

Outreach and Enrollment (O&E)

5. Assist with implementation and coordination of O&E activities, such as, hosting enrollment events, in which may include before or after normal business hours, evenings, and weekends

6. Assist with presentations for community groups and referral sources

7. Attend all required training sessions at the federal, state and local level and meetings concerning O&E

8. Safeguard data, maintain strict confidentiality of information, and perform required reporting

9. Accurately complete data collection and enrollment process

10. Conduct “in reach” with currently uninsured patients and “outreach” with non Bluestem Health patients in all service areas

11. Monitor and report all patient correspondence, including patient/non-patient completed enrollments

12. Develop relationships with appropriate community partners

13. Provide educational materials regarding insurance options to community partners, including: health department, hospitals, urgent cares, other physician offices, and human services agencies and collaborate and coordinate outreach efforts with them

14. Enroll patients in the Medication Assistance Program (MAP), including preparing and submitting applications. 

15. Review patient’s income documentation and apply to sliding fee scale; inform patient(s) of estimated fees for medical and dental services. 

16. In collaboration with members of the health care team, requests orders and/or reorders of MAP medications.

17. Assists in referrals for other agencies, as needed.

18. Performs reminder phone calls to those patients whose sliding fee scale will be expiring on an annual basis.

19. Responsible for processing MAP applications in RX Assist Database. 

20. Responsible for verifying that proper documentation is received, copied, and charted when completing MAP applications.

21. Organize work to meet goals, objectives, and deadlines

Requirements

1. High school diploma, required

2. Degree in a human services field or equivalent experience, preferred

3. Bilingual, strongly preferred

4. Computer skills, required

5. Strong customer service skills, required

6. Friendly outgoing personality which shows compassion and dedication to helping others 

7. Ability to work effectively with underserved and diverse populations

8. Experience in planning and implementing projects and coordination of functions and setting goals and meeting timelines promptly

9. Comply with all applicable federal and state training requirements related to the development of expertise in eligibility, enrollment, and program specifications

10. Professional appearance

11. Tact and patience in dealing with the public

12. Able to work accurately, independently, and as part of a team

13. Must be able and willing to travel as needed, have dependable transportation, a valid driver’s license, and proof of automobile insurance
