The Care Coordinator Manager will:
Facilitate the processing of patient referral orders originating from BCHC providers to outside healthcare providers and/or facilities.
Assist with monitoring and tracking quality and UDS measures.
Facilitate prior authorization for medication and imagining generated by the BCHC providers.
Work with community partners to assist with the needs of patients to improve patient care.
Will be an expert in new processes and be proactive in assisting other care coordinators to embrace and implement new practices.
Monitor patients with chronic illness, documents progress and reports to appropriate Provider barriers to care.
Facilitate all areas of acre assigned to care coordinator and referral coordinator team.
Ensure appropriate workload and division of jobs among teams.
Provide Health Education and Case Management per phone calls, clinic visits and communicate with Insurance Companies to provide all services allowed patients.
Connect to Ky. Medicaid Marketplace, Medicare or other insurance options.
Monitor and address tardiness, call-ins and scheduling.
Perform yearly evaluations and ongoing check ins evaluations.
Serve as clinical liaison to other departments.
Monitor level of tasks to prevent delay in care.
Assist with interviewing and orientation of new staff.
Assist with ongoing competency of skills needed for clinical staff including yearly competency, monthly education and remediation with needed related to care coordination.
Assist conduct monthly department meetings.
Perform chart audits to ensure orders are completed correctly and timely, and charting by staff is accurate.
Identify and communicate problems related to clinical operations to Director of Clinical Operations.
Promote team work and standardized operating procedures for clinic.
Perform all required training, attend all mandatory meetings and reinforce guidelines set forth from meetings with team members.
Provides backup coverage and assistance to other employees as required or requested.
Acts in compliance with contracts, laws, regulations, BCHC policy, procedures and standards.
Completes special projects and other duties as assigned.
Attend conferences and educational session as needed
80% Clinical time/Care Coordination, 20% Administrative time
