Coming home from the hospital or facing chronic health conditions can be challenging. About this role:

The Clinical Care Coach will be responsible for providing coaching services to eligible patients in various environments and locations. The staff will develop a wellness coaching relationship with clients and assist the client towards better health by providing support, encouragement, education, and resources when needed.

• Is a graduate of an accredited school of professional (registered) nurses, or practical nurse or vocational nursing program.
• Is currently licensed as an RN or LPN through the State Board of Nursing.
• Minimum of 1-2 years’ experience in home health, health care or community-based organization.
• Strong communication and organizational skills.
• Knowledge of medical terminology.
• Strong education/teaching background.
• Ability to be flexible and adjust practice based on client population being served.
• Experience in problem solving, managing complex problem, and developing solutions.
• Strong customer service skills and the ability to work independently.
• Ability to exercise judgment and discretion in confidential matters.
• Must be able to make home visits to patients in a variety of settings; mobile homes, apartments, homes, nursing facilities and condominiums that may not have elevators.
• Must be able to drive for extended periods of time with no ill effects.
• Must be flexible, good written and oral communication skills.
• Must have computer access (PRN only) and excellent computer skills (ALL).
• Must have the ability to give and receive constructive criticism.
• Employees will be expected to comply with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.
• Must be a licensed driver with automobile that is insured in accordance with state and/or BCN requirements and is in good working order.
