Overview:

We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide quality care to patients in the comfort of their homes, ensuring their well-being and promoting independence.

Responsibilities:

- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient conditions and update medical records accurately
- Collaborate with physicians and healthcare team members for optimal patient outcomes
- Provide patient education on disease management and self-care techniques
- Perform diagnostic evaluations and assist in discharge planning

Requirements:

- Valid RN license in the state of practice
- Minimum of [Critical care], [Behavioral health], or [ER] nursing experience preferred
- Proficiency in [Medical records] management and [Utilization management]
- Knowledge of [HIPAA] regulations and patient privacy practices

Pay: $30.00 - $35.00 per hour
