Job Requirements :

Must Be HHA Certified

MUST SUBMIT :

• Valid original NYS HHA / PCA certificate
• Valid I-9 Information to establish both identity and employment authorization
• Two (2) Reference letters - One Personal and One professional
• Pre-employment physical - Within the last 12 months.
• Negative PPD, Chest X-Ray (if PPD positive)
• Rubella / Rubeola and Drug Screen (Lab-work attached).
