Med Tech, Direct Support Professional, Direct Care Worker, Caregiver - New Baltimore, Michigan (Full Time) – Licensed Group Homes
This Program offers community living in family settings. All homes are supervised 24-hours a day by trained Direct Support Professionals, Direct Care Workers, Caregivers.
Shift:
full time positions, 40 hours per week, shifts vary
Rate of Pay (commensurate with experience and education):
Up to $21.04 per hour including State of MI premium, dependent upon education and experience.

Job Description:
We are seeking Med Techs, Direct Support Professionals, Direct Care Workers, and Caregivers who will focus on improving the physical and mental well-being of individuals with special needs, while maintaining a positive and safe living environment for the individuals served and monitoring/reporting on their progress.
  • Transport those served to/from appointments and outings.
  • Accurately maintain logs, files, records, notes and all documents required by the organization.
  • Attend and participate in staff meetings and on-going trainings.
  • Immediately report any policy violation or any suspected abuse or neglect to their immediate supervisor as required by Section 400.11a of the Social Welfare Act 280 of 1939.
  • Implement treatment plans as designed by the interdisciplinary team.
  • Work cooperatively with management, team members, parents, guardians, and co-workers to maintain a proper environment within the group home.
  • Participate in any/all tasks necessary to efficiently operate the home (i.e., cooking, laundry, lawn care, shoveling, cleaning, home maintenance, vehicle care and maintenance).
  • Maintain confidentiality regarding those served by the organization and regarding co-workers.
  • Administer medications as directed by the physician.
  • Document height quarterly.
  • Document weight monthly.
  • Label individual resident health care logs.
  • Order/reorder prescriptions on a regular basis.
  • Maintain the security and organization of the medical cabinet.\Complete and file all inactive or out-of-date medical documentation per established procedures.
  • Maintain monthly medication logs, ensuring that each month, the type of medication, the dosage of medication and the time given are printed or typed clearly and accurately. (Remember to have both last month’s medication chart and the bottle with the pharmacy label available when doing the recent medication log.)
  • Develop a yearly checklist for each resident identifying dates of routine medical/dental services (i.e., annual physicals, six (6) month dental checkups, T.B. tests, six (6) month blood levels for seizure medications).
  • Maintain an ongoing audit of routine health care and schedule visits, as necessary.
  • Maintain a prescription record to document staff ordering and pick up of prescriptions.
  • Arrange, send, and document medications sent to school, day program or on an outing.
Qualifications:
  • High school diploma or equivalent is required.
  • Acceptable criminal record check and drug screen.
  • Eighteen (18) years of age or older.
  • 1500 hours experience working with individuals having an emotional and/or mental impairment, or 30 credit hours of college in a related field.
  • Maintain reliable transportation and valid driver’s license with a driving record acceptable for insurance purposes.
