As an MDS Coordinator / RN Assessment Coordinator with us, you will play a crucial role in ensuring that our patients receive the care they need and that our facility operates in compliance with federal and state regulations.

Job Description :

As an MDS Coordinator / RN Assessment Coordinator, you will be responsible for overseeing the accurate completion of the Resident Assessment Instrument (RAI) in accordance with federal and state regulations and company policies.

You will act as an in-house case manager, coordinating services with physicians, families, third-party payers, and facility staff to ensure comprehensive care for our residents.

Your critical thinking skills and attention to detail will be essential in developing appropriate care pathways and communicating timely information to all stakeholders.

Key Responsibilities :

Ensure accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments (CAA's), and Care Plans in accordance with current regulations and guidelines.

Act as an in-house Case Manager, considering all aspects of the resident's care and coordinating services with relevant parties.

Communicate proactively with administrators and nursing directors to identify regulatory risks, assess facility systems' effectiveness, and address clinical trends affecting resident care.

Collaborate with therapy staff to select appropriate reasons for assessment and Assessment Reference Dates (ARDs), and capture relevant RUG scores reflecting care and services provided.

Demonstrate understanding of MDS requirements for various payers, including Medicare, Managed Care, and Medicaid.

Ensure timely electronic submission of all Minimum Data Sets to the state database and follow up on validation reports.

Facilitate the Care Management Process to identify and resolve barriers to discharge, ensuring optimal resident outcomes and safe transitions.

Analyse Quality Improvement / Quality Measure (QI / QM) data with the Director of Nursing Services to identify trends and inform decision-making.

Complete Utilization Review and Triple Check processes in a timely and accurate manner.

Participate in various facility committees and serve as a backup user for NHSN reporting.

Fulfill other administrative duties as assigned.

Qualifications :

Registered Nurse with current, active license in the state of practice.

Minimum of two (2) years of clinical experience in a healthcare setting.

Minimum of one (1) year of experience in a long-term care setting.

Prior experience as an MDS Coordinator preferred.

Training program available for RN candidates with demonstrated assessment skills.
