Blue Ridge Independence at Home is Hiring a Registered Dietitian Near Winchester, VA

Blue Ridge Independence at Home is seeking qualified applicants for a fulltime Dietitian.

Blue Ridge Independence at Home (BRIH) was established to deliver PACE (Program of All-Inclusive Care for the Elderly) services. PACE's vision is to provide individualized and joyful care through exemplary teamwork serving the best quality-of-life care to seniors in our community.

OBJECTIVE: Screens and assesses member nutritional status. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the same to the inter-disciplinary team to ensure the nutritional needs of members are met, including those members who are considered to be at high nutritional risk. Assists in kitchen/kitchenette inspections and in-services to meet Nutrition and Dining Services goals of sanitation and education. Conducts group nutrition education to members as assigned.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1.Must have a valid driver’s license, proof of insurance and have means of transportation.

2.Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

3.Education - Bachelor’s degree or advanced degree in food and nutrition or dietetics from an accredited college or university, current American Dietetic Association registration and state certification required.

4.Experience - A minimum of two (2) years’ experience in working with the frail or elderly population or if the individual has less than two (2) year of experience but meets all other requirements... must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. Experience working on a multi-disciplinary team in a hospital, nursing home or community-based setting is preferable.

SKILLS AND KNOWLEDGE:

·Ability to read, speak and write fluently in English.

·Working knowledge of physical, mental, and social needs of disabled and older adults.

·Effective skills in assessment of nutritional needs of disabled and older adults.

·Effective written and oral communication skills.

·Ability to work with an interdisciplinary team, as well as independently.

·Demonstrates necessary skills and knowledge as needed for position-specific Competency Assessment Profile

·Training and/or mentoring experience and ability to complete performance objectives, measures, and evaluations.

ESSENTIAL FUNCTIONS:

1. Conducts a comprehensive nutritional assessment on PACE members upon enrollment and a periodic nutritional review on members as scheduled.

2. Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participation in, interdisciplinary team meetings; collaborates on care planning and nutrition related service allocation decisions.

3. Provides direct guidance to CNA and assists with assigned members’ nutritional care.

4. Documents nutrition notes in the member’s electronic medical record.

5. Develops and implements nutrition care plans for high-risk members including medical therapy recommendations.

6. Facilitates and participates in Nutritional Improvement Program (NIP) reviews as scheduled.

7. Provides printed nutrition notes on members for members residing in residential facilities.

8. Develops and implements the nutritional components of member care plans including active participation and development of therapeutic dining programs.

9. Responsible for the management of therapeutic diets, including modifications of the general menu to meet individual member needs and physicians’ orders.

10. Assists in monitoring meals, including intake, diet compliance and tray accuracy.

11. Calculates data obtained from food intake records and calorie counts and makes recommendations for changes to appropriate staff.

12. Provides instruction to members and caregivers regarding dietary regimens and nutritional requirements. This includes home, group home and nursing home visits, when necessary.

13. Maintains supplement and bag lunch list for assigned site(s).

14. Monitors appropriate supplement use for all members according to department guidelines.

15. Monitors sanitation of food preparation and kitchen area at assigned site(s) through planned and unannounced inspections.

16. Assists in the training of dietary and clinical nutrition staff as assigned.

17. Participates in and provides in-service education to employees and members as requested.

18. Assists in development of policies and procedures for nutrition services.

19. Participates in Member Council, reports findings, and recommends follow-up., as assigned.

20. Works cooperatively and harmoniously with all staff and members.

21. Maintains professional demeanor with families and visitors.

22. Protects privacy and maintains confidentiality of employees, member, and sensitive agency information.

23. Attends department meetings and in-services, as well as all mandatory agency in-services.

24. Keeps current with computer documentation training and education.

25. Performs other duties/projects and participates on other committees as assigned.

26. Maintains continuing education credits required by the American Dietetic Association.

27. Performs other duties as required or requested.

1. Physical Requirements – Shall be in good mental and physical health, and capable of performing assigned tasks. Requires standing, walking, occasional pushing and pulling and lifting. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination.

2. Visual, Hearing and Communication Requirements – Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to document care provided in participant records. Must be able to communicate verbally with participants, family, and team.

3. Pressure Factor – Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuation indoor temperatures. Required to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression as well as difficult family dynamics, including caregivers under stress.

5. Know the location and purpose of Safety Data Sheets (SDS).

6. Follow guidelines for department environment and safety measures.

7. Follow guidelines for safe handling, inspection, maintenance, and storage of equipment. Report any malfunction of equipment.

8. Follow guidelines for safe handling and storage of chemicals and supplies.

9. Follow correct policy and procedure for reporting of incidents: staff, residents, etc.
