Description

Position Summary:

The Palliative Practice Social Worker enables Blue Ridge Palliative Care Services to function more effectively by supporting the palliative care medical staff in providing direct care to patients and families through SW support with goals of care review and coordination of care needs. Additionally, this position will have an administrative role that will involve coordination with the administrative assistant to ensure effective patient scheduling, data collection and policy development as the service line grows.

Responsibilities/Essential Functions:

  1. Serves as Palliative Team social worker for field patients in private residences and facilities.
  2. Possesses interpersonal skills that promote collaboration and effective communication.
  3. Manages time effectively to encompass daily management of Blue Ridge Palliative Care Services.
  4. Demonstrates ability to maintain business files in a systematic and orderly fashion.
  5. Demonstrates a working proficiency in the operations of Medicare, Medicaid, and other pay sources in collaboration with billing department for accounts receivable management.
  6. Demonstrates data collection and analysis skills.
  7. Acts independently and responsibly to perform job duties on a consistent basis.
  8. Sensitive to stressors experienced by colleagues.
  9. Seeks opportunities for professional growth, including demonstrating the ability to receive constructive performance feedback.
  10. Must have a team mentality, flexibility and willingness to learn, interpersonal skills and customer orientation.
  11. Demonstrates a commitment to quality patient care.
  12. Provides SW support in the private home setting and facility setting as needed to palliative care patients and families. These responsibilities include, but are not limited to, goals of care discussions, completion of DNR, and POST forms, advance directive review as needed and coordination of care among existing providers. This will be in close coordination with BRPCS NP and physician.
  13. Assists with communications with patients, referral sources and external providers
  14. Schedules patient initial and follow-up visits with providers
  15. Establish and update BRPCS policies and procedures, ensuring compliance with all regulatory guidelines along with CMO.
  16. Develops and updates BRPCS forms as needed.
  17. Ensures contracts/ licensures/credentials/privileges for all BRPCS clinical staff remain current with Medicare, Medicaid, Commercial Insurance, and Hospitals.
  18. Contributes to the annual budget development for BRPCS.
  19. Provides administrative supervision to BRPCS administrative assistant and administrative volunteers.
  20. Ensures that BRPCS promotional tools and printed materials are produced, updated periodically and disseminated to the appropriate audience.
  21. Provides statistical, financial, and other data reports with the assistance of administrative assistant.
  22. Coordinates response to requests from the community for informational presentations, ensuring a professional and consistent message.
  23. Develops and creates Power Point presentations specific to target audiences.
  24. Responsible for all billing/ account activity, acts in accordance with policies and procedures in coordination with billing department.
  25. Coordinates and schedules educational opportunities for BRPCS staff and community audiences as appropriate.
  26. Participates in educational and networking opportunities that foster growth and development of community services.
  27. Close participation in recruitment and hiring of new members of BRPCS team.
  28. Respects the autonomy and right to confidentiality to all patients.
  29. Must be willing to travel within our service area to provide services.
  30. Seeks opportunities for professional growth, including demonstrating the ability to receive constructive performance feedback. Seeks assistance of supervisor or other resource persons in responding to opportunities for improvement.
  31. Participates in departmental and/or agency wide quality improvement activities.
  32. Acts independently and responsibly to perform job duties on a consistent basis.
  33. Carries out the mission of Blue Ridge Palliative Care Services.
  34. Other duties as assigned.

Education and Experience:

  1. Bachelor of Science in Social Work. LCSW required.
  2. Prior experience with practice management.
  3. Excellent verbal and written communication skills
  4. Strong leadership and organizational skills
  5. Experience preparing and presenting educational workshops to community groups.
  6. Skills in decision making, program planning, development, administration and evaluation are required.
