Responsibilities:

• Following the resident's individual care & service plan in providing one-on-one support and companionship: including daily wellness checks and housekeeping;
• Assisting residents to the bathroom, providing assistance and hygiene as needed
• Supervising with activities of daily living: including bathing, dressing, eating, toileting, and hygiene
• Escorting residents to activities, meals, nature walks, and outside appointments as needed
• Assisting in the development and updating of each resident's individual service plan and charting
• Encouraging socialization and participation in community activities
• Cleaning the assigned area for patient’s needs 
• Monitoring residents every two-hour bed checks, staff will need to help with Activities of Daily Living
