Intensive In-Home Services Department

Job Description & Responsibilities:
Associate Professional

Job Title: Associate Professional; AP

Direct Supervisor: Qualified Mental Health Professional

The program shall be under the direction of a person who meets the requirements specified for QP status according to 10A NCAC 26C .0502 and .0504.

Supervises others: NO

IIH Associate Professional (AP)

Intensive in-home service means family preservation interventions for children and adolescents who have or are at-risk of serious emotional disturbance, including individuals who also have a diagnosis of intellectual disability. Intensive in-home service is usually time-limited and is provided typically in the residence of an individual who is at risk of being moved to out-of-home placement or who is being transitioned back home from an out-of-home placement. The service includes 24-hour per day emergency response; crisis treatment; individual and family counseling; life, parenting, and communication skills; and case management and coordination with other services.

The Intensive In-Home Services Associate Professional (AP)

Job Responsibilities:

Daily:

Provide direct care to clients within the treatment setting

Participate in updating of consumer's PCP/Treatment Plan with other team members

Interact with parents/guardians as necessary

Provide psychoeducation (information about consumer's diagnosis and effective interventions) to parent/caregiver, school personnel, other agencies involved with consumer

Interact with school personnel regarding consumer's schedule/behavior/etc.

Assist consumer and family in developing a behavior modification plan to be implemented in the home, school, community

Monitor and document consumer's progress and effectiveness of strategies and interventions outlined in the person-centered plan.

Coordinate and oversee the initial and ongoing assessment activities

Weekly:

Meet with team members on each assigned case and update team on consumer's progress/regression/stagnation

Assist with coordination of care for consumer

Provide interventions in the home, school (with parental permission), and community

Provide in-home interventions to consumer and/or family

Obtain direction from team lead/case manager regarding appropriate interventions for consumer

Participate as a first responder team member in crisis management cases (as appropriate) 24/7/365 days

Participate in meetings involved with consumer (DJJP, school, DSS, other service providers)

Monitor the provision of other services to determine if these services are effective

Attend weekly team meeting to provide consumer updates

Monthly:

Participate and contribute to CFT’s. Provide information about consumers with concrete examples

Provide team with updated consumer information/behavior to update the PCP, crisis plan, clinical summary, and discharge summaries

Provide justification for adding, modifying, and/or discontinuing any and all goals

Attend monthly supervision

Annually:

Provide information to the team to complete the annual PCP renewal

Provide information to the team to complete the annual Crisis Plan

General:

The employee will complete documentation of sessions with clients within 72 hours of the encounter

In the event the caseload decreases, the employee agrees to perform other clinical duties to justify compensation

At least 60% of the contacts shall occur face to face with the youth, family, and caregivers or outside of the agency's facility, with or on behalf of the recipients. The remaining units may be completed per staff/guardian/consumer preference.

Participate in any agency audits as required

Participate in any required trainings

Major Responsibilities:

· Provide behavioral support to consumers and families within the community and home environment.

· Ensure case management services are provided for each consumer.

· Assess each beneficiary's needs for comprehensive services.

· Develop and implementing the PCP.

· Convene Child and Family Team meetings to coordinate the provision of multiple services and the development of, and revisions to, the PCP.

· Link the beneficiary or family to needed services and support.

· Monitor the provision of services and supports. Assessing the outcomes of services and supports.

· Collaborate with other medical and treatment providers.

Collaborate with other behavioral health service providers, as well as with identified medical (including primary care and psychiatric) and non-medical providers (e.g., Dept. of Social Services, school, Dept. of Juvenile Justice), engages community and natural supports, and includes their input in the person-centered planning process.

Knowledge, Skills and Abilities:

Ability to communicate effectively, verbally and in writing’ Ability to establish effective working relationships with families, coworkers, supervisors, and other individuals, Possess effective interpersonal skills, Knowledge of the agency's organizational structure, standard operating procedures, and policies, Knowledge of intervention and behavior management methods, strategies, and techniques, Knowledge of quality documentation as required by agency standards, rules, and regulations, Ability to work as a multi-disciplinary team member in a positive productive manner, Self-awareness and understanding of professional boundaries. Knowledge of factors which may contribute to family dysfunction; knowledge of behavioral management techniques; general knowledge of community resources and services; knowledge of crisis intervention strategies; good problem solving skills; ability to maintain effective working relationship with other social services providers, service purchasers, and clients.

Educational and Experience Requirements:

The Associate Professional must meet ONE of the following qualifications based on the targeted population being served:

ALL Staff providing Intensive In-Home Services to children and families must have a MINIMUM of one (1) year documented experience with this population.

Bachelor's degree in a human service field with less than two years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional (with a bachelor's degree in a human service field) with less than two years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling, OR

Bachelor's degree in a field other than human services with less than four years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional (with a bachelor's degree in a non-human service field) with less than four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling, OR

Registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing with less than four years of full-time accumulated experience in mh/dd/sa with the population served

Job Types: Full-time, Part-time

Pay: $15.00 - $17.00 per hour
