Home Health and Hospice Medical Social Worker.

We are currently seeking qualified and professional Full-Time Medical Social Workers to join our Home Healthcare agency.

Job Description:

Our Home Healthcare agency services areas across all counties of the Bay Area, but we are specifically in need in the San Francisco and San Jose area. We have a current need for MSWs who are eager and able to work in the Bay Area.
We provide a comprehensive care program to each of our patients.

Medical Social Worker:

Responsible for facilitating patients and families in the identification and management of bio-psycho-social, spiritual, financial, development, and cultural issues in the face of acute, chronic, or terminal illness to keep them safe and adequately cared for at home.

Requirements Include:

Must have graduated from an accredited school with a degree in social work.
Current CPR certification.
Develop a plan of care for each assigned patient.
Assesses patient needs, plans, evaluates, and modifies care to meet the goals of social work interventions.
Collaborates with all disciplines to plan and evaluate team goals for each patient.
