We're looking for a Registered Nurse Case Manager/Field Nurse who can help us provide the highest quality care to our patients in their homes. The Registered Nurse Case Manager will be responsible for planning and coordinating patient care, working with patients' physicians to make sure that treatment goals are being met and communicating regularly with members of the clinical team. RN will also be meeting with clients to do start of care, recertifications and will be working to input information in our online system. We're seeking an individual who understands the needs of our patients and their families, and is capable of developing a plan of care that keeps everyone involved in treatment decisions. This position could lead to the Director of Nursing position.

Skilled Nursing is the same as office rate which is Hourly

A few office hours are needed per week to help input patient information into our online system.

Responsibilities:

• Manage discharge planning and follow-up with patients and family members after discharge.
• Skilled Nursing in the clients home.
• Supervise LPN's and Home Health Aides in the clients homes
• Communicate with Physicians
• Ensure proper documentation
• Start of Care
• Recertifications every 60 days
• Provide the HHA's with a Care Plan of clients needs

Job Types: Full-time, Part-time

Pay: $35.00 - $60.00 per hour

Expected hours: 10 – 30 per week
