Job Description: Social Worker-Bledsoe County Schools-Stronger Connections Grant Funded

The social worker will work with the grant director, school counselors and the social services team to further develop and maintain a written plan of data-driven goals and strategies for effective delivery of the school social work program. The social worker will focus on addressing the needs of students at elevated risk for depression and related mental health disorders. The social worker will demonstrate excellent planning, organizational, problem-solving, decision-making, team-building and time management skills. The social worker will also work to build bridges between school, home, and the community by providing wrap-around services to students and families to address barriers to student achievement. The social worker will be responsible for case management /assessment and provide on-going documentation. Duties and responsibilities include counseling, assisting in the development of behavior plans, assisting administration and staff with behavior interventions, monitoring student behavior, creating appropriate student records, and assist with students displaying at risk behaviors.

Responsibilities: The following duties are normal for this job. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

Participate in Student Success Planning team meetings. During these meetings school social worker will consult and problem solve with staff as part of the Multi-Tiered Systems of Support (MTSS/Student Planning Team).

Develop and assist in the implementation of plans for mental health interventions with students that may include Crisis Intervention, Suicide Risk and Threat Assessments

Assist/Lead/ participate in the district crisis response team providing crisis management (individual and school wide crisis prevention, response, and recovery)

Complete risk assessments for students who have made statements of a suicidal or homicidal nature/ participate in re-entry plans according to school protocols.

Provide individual/group work services/counseling to students using scientifically researched based interventions to enhance their behavioral skills, coping strategies, and reduce symptoms in the school environment.

Provide conflict resolution strategies and anger management for students as needed

Maintains up-to-date, accurate student records/caseload and AWARE referral pathway management

Regularly evaluates the effectiveness of social work services and identifies need for additional services.

Responds to internal and external customers in a timely, accurate, courteous, empathic, and professional manner representing Bledsoe School System in a positive light

Provide case management and wrap around services to students and families, this includes outside referrals to maximize access to community- based agencies, school mental health partners and practitioners. Act as a liaison between these groups to ensure that comprehensive services are provided to students and their families

Obtains releases of information for outside therapists, hospitals, and service providers as relevant to coordinate services and create an appropriate and individualized

Provides staff development activities for school personnel on topics such as abuse, neglect, developmental delays, emotional disturbances.

Provide trauma informed practices and restorative circles support and training to school and district level staff

Interprets criteria for community resource utilization to families and assists in locating appropriate services outside the school when needed.

Provides support and counseling to families. Provides training in parenting skills.

Provides information to parents (when appropriate) about social needs and problems of students.

Reports student progress and status regularly to parents.

Additional Job Functions:

Must complete training in job specific activities required by the grant and Bledsoe County Schools.

Attends work and arrives to work on time

Adheres to the Social Work Code of Ethics and the Teacher Code of Ethics as defined by Tennessee Law; Maintains confidentiality

Maintains appropriate licensure and acknowledges that continued employment with the Bledsoe County Department of Education is contingent upon providing required documentation to validate licensure in a timely manner

May operate a motor vehicle to conduct various work activities

Evaluations: Reports to SCG grant Director
