About this positionThe Nurse Navigator Oncology Breast Cancer provides care coordination and guides breast cancer customers through the healthcare system and helps them to overcome barriers to optimal care. Responsibilities include customer/family education, customer advocacy, support service coordination, communication facilitation, and multidisciplinary participation. The Nurse Navigator Oncology Breast Cancer is a constant, familiar face that is there to answer questions, reinforce customer education, ease anxiety, and act as a resource for both the customer and their family. In addition, this navigator serves as a liaison for multiple disciplines including surgery, oncology, radiation oncology, pathology and radiology. What we'll expect from you* Demonstrates effective communication with customers, families, team members, health professionals, organizational leaders, and other key customer groups. * Assists customers in accessing cancer care and navigating healthcare systems. * Provides customers and families evidence-based information and refers to provider for questions about clinical information, treatment choices, and potential outcomes. * Demonstrates familiarity with and know how to access and reference evidence-based information regarding cancer screening, diagnosis, treatment, and survivorship. * Collaborates with physicians and other disciplines to provide comprehensive clinical coordination of care including: Primary Care MD, Surgeons, Plastic Surgeons, Oncologists, Radiation Oncologists, Radiologists, Pathologists, and Nursing. * Serves as Breast Cancer resource for organization and community. Facilitates Breast Cancer support group and attends weekly Breast Cancer tumor conferences. * Educates customer and families about the recommended treatment, side effect management, and survivorship/end of life care. * Monitors customers who are experiencing toxicities/side effects to treatments and serves as conduit between customers and providers. * Effectively manages resources and leverages community resources to assist customers. * Provides expert consultation for customers, staff, and colleagues regarding questions concerning prosthetics, wigs, lymphedema, cold capping, support groups, community programs, etc. * Develops customer and family relationships with emotional support and encouragement. * Empowers customers to make informed medical decisions. * Maintains clinical knowledge of Breast Cancer guidelines, new treatments/therapies, etc. to ensure navigation services are based on evidence and best practice. * Accepts other duties as assigned to promote the accomplishment of organizational goals. * The team member is accountable to demonstrate proficiency for the skills outlined in the appropriate position skills list. * The team member is accountable to maintain skill proficiency, including improvement where deemed necessary, and upgrading any additional or new skills on the appropriate position skills list. Knowledge, Skills and Abilities * Demonstrated ability to manage significant customer population on a weekly basis. * Demonstrated broad clinical knowledge of breast cancer. * Strong leadership, verbal, written and interpersonal communication skills. * Demonstrated skill proficiency in computer literacy or use of variety of software programs (e.g. Excel, PowerPoint, etc.)Licensure/Certification(s) Mandatory * Current licensure as RN in State of MN State Requirement * Current BLS Certification State Requirement * Must become certified as an oncology nurse through the Oncology Nurse Certification Corporation within one year of date of hire and maintain certification throughout employment in this position.
