Organization Description:

Black River Health Services is a Federally Qualified Health Centre Look a Like, with 3 clinic locations in Pender County. Manos Unidas is a health outreach program serving migrant and seasonal farmworker population through the provision of health services, care coordination, enabling services and health education. The goal of Manos Unidas is to improve the health of migrant and seasonal farmworkers and their families.

Job Role Description:

This role is a hybrid role and covers 2 areas of the outreach program, services coordination and outreach work.

Services coordination implements farmworker dental days and optometry clinics. This includes scheduling, calling, and confirming appointments. Ensuring that all dental and optometry referrals to specialty services are completed. As part of this role, all MAP applications are completed in a timely and efficient manner and PM profiles are completed. This role serves as front desk for the mobile clinic.

The outreach role provides health care outreach and education to migrant camps and seasonal family homes in Pender, Duplin, Sampson, Blden, parts of Jones and Onslow counties. As part of these visits, health assessments, provides education and initiates care coordination. Some enabling services include interpretation during specialty and dental care appointments, scheduling transportation, checking blood pressure, delivering food and completing financial assistance applications.

Responsibilities:

• Work under the supervision of the Program Director.
• Coordinate virtual visits to seasonal family homes.
• Complete health assessments, provide health education, hand out toiletry bag and check blood pressure during outreach visits.
• Demonstrate knowledge of priority health topics including but not limited to Pesticide Safety, Heat Stress, HIV, DWI, 911 and Nutrition.
• Provide care coordination services such as initial assessment, referral, tracking and follow ups.
• Document all farmworker encounters on appropriate forms with detailed notes and enter into FHASES database.

Qualifications:

• Must be fluent in both Spanish and English.
• Minimum of a high school education.
• Cultural competence with Hispanic/ Latino populations including indigenous populations from Mexio and Central America.
• Ability to work independently as well as part of the wider outreach team.
• Must be flexible with working hours.
• Must be comfortable with working in a challenging and overcrowded, substandard housing environment.
• Must hold a valid driving license.
• Must have reliable transportation.

Job Type: Full-time

Pay: $19.00 - $22.00 per hour
