Patient Care Coordinator

Qualifications:

Essential Duties and Responsibilities
  • Cordially meet with and provide direction for patients and guests.
  • Promptly responds to calls/mail in a professional, courteous, and helpful manner.
  • Maintain filing system for patient after care needs and possibilities.
  • With the assistance of the clinical care team and the patient; formulate Aftercare Plans that will ensure best possible outcomes
  • Completes the Referral and Intake Processes accurately as it to pertains to referrals to aftercare referral source
  • Meet with business development representatives and referral sources to ensure PCC has resources across the country that meet patient needs
  • Observes all confidentiality regulations.

Education and Experience
  • Bachelor’s degree in psychology, social work, or related field required. Master’s in a related field preferred.
  • 2 years’ experience in a mental health/healthcare field preferred. Insurance verification and/or insurance billing experience are a plus.
