Job Description:

• Travel may be required in any of the following: Bergen, Essex, Hudson, Hunterdon, Middlesex, Morris, Passaic, Somerset, Sussex, Union, Warren.
• Field-based RNs conduct clinical eligibility assessments for individuals seeking Medicaid eligibility who reside in community-based settings or nursing facilities statewide.
• Travel is required for this position.
• A background check for motor Vehicles is required as well as a personal vehicle.
• The nurse will be responsible for reviewing medical records, interviewing individuals and completing a comprehensive standardized assessment tool within hospital, nursing facilities, assisted living, senior housing, and private community homes.
• The nurse will also be responsible for providing counseling on service options.
• Training and mentoring is provided over a span of 4-8 weeks.
• Training sessions may be virtual or in-person in the regional field office.

Work hours:

• Shift Timing: 08:30 to 04:30
• Lunch: 1 hour (unpaid)

Additional information:

• Primarily remote with daily travel to community/facility settings in the counties listed above.
• Office attendance may be required for monthly in-person meetings/trainings.

Behavior Characteristics:

• Communication Skills; info gathering & documentation. Interpret rules and follow established processes.

Pay: $52.74 per hour
