Billings Clinic - Health System is Hiring a Total Joint Program Coordinator Near Billings, MT

The incumbent in this

position is responsible for the development, implementation, and evaluation of

the total joint program at Billings Clinic. This individual works in close

collaboration with the total joint medical director to organize the services and

systems necessary for a multidisciplinary approach to care for total joint

patients. Works collaboratively with the health care team, including

physicians, nursing, case management and other health care providers to improve

the physical, psychosocial, rehabilitative, educational needs of the total

joint patient and/or their family. Responsibility for the day-to-day process

and performance improvement for total joint program rests with this position.

The role includes quality tracking, educational, clinical, administrative, and

community outreach activities for the total joint program.

Essential Job Functions

Supports and models behaviors

Maintains knowledge and adheres

to standards and criteria set forth for Total Joint Program Certification

and Accreditation.

Coordinates the care of total

joint patients in conjunction with the surgeon and team. Collects,

reviews, and analyzes information for evaluating patient care outcomes.

Maintains the total joint

dashboard for the purpose of performance improvement by measuring and

evaluating plans to improve both the processes of care and the outcome.

Supervises the collection,

coding, scoring, and development processes for validation of data.

Facilitates in designing the

dashboard to facilitate performance improvement activities, trend reports,

and quality metrics while protecting confidentiality.

Coordinating total joint care

management utilizing effective communication across the continuum of care,

including planning & implementation of clinical protocols/practice

management guidelines, monitoring care of in-hospital patients, serving as

a resource for clinical practice, and discharge planning.

Identifies multidisciplinary

opportunities for improvement in a timely manner, and assists team members

with problem resolution through plan development, implementation and

evaluation.

Monitors quality, cost,

continuity of care, clinical outcomes and system issues related to care delivery,

developing quality filters, audits, and case reviews, identifying trends

and sentinel events and helping to outline remedial actions while

maintaining confidentiality.

Assists in coordinating the

delivery of services to facilitate the patient’s progression through the

continuum.

Is responsible for the

operational aspects of the Total Joint Program, representing the Total

Joint Program on various hospital and community committees to enhance and

foster optimal care management.

Facilitates and ensures that

comprehensive, multidisciplinary collaboration of total joint care occurs

with the patient, family, physicians and other health care providers.

Identifies

fragmentation/duplication and assists the team in defining effective

utilization of resources.

Coordinates and recorder for

total joint committee and multidisciplinary total joint peer review

meetings.

Provides and supports total

joint education for Billings Clinic staff and assists in inter-facility

and regional staff development.

Participates in case review,

standardizing practice guidelines, monitoring educational requirements and

assisting in data gathering for surgical outcomes.

Stabilizes the complex network

of many disciplines that work together to provide high-quality care,

serving as an internal resource for physicians and staff in all Billings

Clinic departments, and acting as a liaison for the total joint program.

Works with Total Joint Medical

Director to develop and evaluate evidence-based guidelines, pathways, and

protocols to reduce inappropriate variation in care. Is involved in,

analysis, and distribution of findings, and facilitates protocol design

for accurate data collection, feedback, and analysis.

Identifies needs and sets goals

for own growth and development; meets all mandatory organizational and

departmental requirements.

Exhibits exceptional knowledge

in the total joint patient’s clinical care. Maintains competency in all

organizational, departmental and outside agency environmental, employee or

patient safety standards relevant to job performance.

Performs other duties as

assigned or needed to meet the needs of the department/organization.
