We are looking for a RN experienced in MDS to join our 5-star community. This role will be focused on MDS and a part of our nursing management team. Our health center has 60 beds with 12 being Medicare certified. This role is also Monday - Friday, 9am - 5pm.

Summary

As the MDS Nurse at Hoosier Village, you are responsible for the accuracy and completion of the MDS Assessments. The MDS Nurse plays a crucial role in ensuring quality care delivery and regulatory compliance within our community. This is a Monday through Friday position, with 8-hour shifts. 

Duties and Responsibilities

• MDS Assessment and Documentation
• Care Planning and Coordination
• Quality Assurance and Compliance related to the MDS
• Provide education and training to nursing staff on MDS assessment protocols, documentation requirements, and regulatory guidelines.
• Serve as a resource for staff inquiries regarding MDS coding, documentation best practices, and care planning processes.
• Take part in the nursing on-call schedule with a $100 bonus for when called-in.

Qualifications

• Active RN license in the state of Indiana
• Active LPN license in the state of Indiana 
• Preferred: certification of MDS assessment (RAC-CT)
