Nurse Care Manager

POSITION SUMMARY: The Nurse Care Manager provides direct and indirect nursing care and medical service coordination to adults, adolescents and children coping with behavioral health and substance use disorders at outpatient clinics and/or residential sites.

Accountable and responsible for the following results:

  • Provide person-centered medical and psychiatric nursing care to clients as indicated and directed by medical and psychiatric providers, and/or Chief Medical Officer (CMO).
  • Demonstrate an understanding of person-centered, harm reduction principles in carrying out job functions and client care.
  • Provide care management services to assist clients in meeting their health and wellness goals.
  • Perform physical health assessments, fall risk assessments, obtain vital signs, and administer other health and risk screening tools as necessary. Communicate negative findings to applicable providers and staff for follow up.
  • Monitor OUD withdrawal symptoms utilizing the Clinical Opiate Withdrawal Scale (COWS) and the Subjective Opiate Withdrawal Scales (SOWS).
  • Monitors alcohol withdrawal symptoms utilizing the Clinical Institute Withdrawal Assessment for Alcohol (CIWA) and the Prediction of Alcohol Withdrawal Severity Scale (PAWSS).
  • Perform drug screenings and breathalyzers as indicated.
  • Monitor blood-draw status and laboratory data specific to clients as indicated. Report pertinent information to prescriber regarding abnormal levels or inconsistent blood-draws.
  • Administer injectable medications as ordered by Medical and/or Psychiatric Providers.
  • Provide oversight of medication administration, monitor response and medication effectiveness for clients at residential settings.
  • Conduct health education with clients, families and other natural supports to increase clients’ understanding of health conditions, symptoms and disease management.
  • Coordinate with the interdisciplinary team to develop an integrated treatment plan that addresses all aspects of health; physical as well as behavioral, that includes person centered goals.
  • Coordinate with primary care and specialty providers to ensure timely and appropriate care for clients as indicated; facilitate referrals to PCP or other specialty providers as needed and/or assist with scheduling appointments; communicate with providers as needed; monitor ongoing primary and specialty care needs.
  • Coordinate with Quest Diagnostics and / or similar laboratory to ensure clients follow-up with necessary lab work.
  • Collaborate with home health care agencies, specialty providers and hospitals to facilitate coordinated transitions of care.
  • Provide health education and/or other relevant consultation to treatment teams and residential counselors.
  • Provide individual and group health counseling to clients, families and/or other natural supports as needed.
  • Facilitate linkage to self-help and support services.
  • Assist with community education and outreach activities.
  • Assist with inspection of necessary medical supplies and equipment and communicate stocking needs to appropriate administrative support staff.
  • Demonstrate competency for providing person-centered services to clients with co-occurring disorders including an understanding of the Integrated Dual Diagnosis Treatment (IDDT) principles, and the ability to utilize Motivational Interviewing (MI).
  • Responsible for accurate, timely completion of necessary documentation.
  • Responsible for assisting in the achievement of program / service specific Outcome Measures.
  • Ensures compliance with oversight body requirements and regulations (Joint Commission, DMHAS, SAMHSA, Medicaid, Medicare, etc.).
  • Attend training sessions, workshops, seminars as requested by supervisor, or as needed forprofessional development, or as required for accreditation or licensing.
  • May be required to use own vehicle on company business.
  • Perform other assignments and duties as assigned.
  • Regular and consistent attendance is an essential function of this position

Qualifications


  • Current Connecticut RN License.
  • Bachelor’s Degree preferred.
  • 2-3 years nursing experience in a clinical setting required.
  • Minimum 1 year of relevant nursing experience with behavioral health required. Substance use disorder treatment experience preferred.
  • Strong communication and documentation skills.
  • Sensitivity to diverse cultures and language competencies.
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
  • Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.

PROFESSIONAL LICENSURE /CERTIFICATIONS: 

  • Current Connecticut RN License.
