Case Manager (Coach – Level 2)
BeyondHome (Colorado Homeless Families, Inc dba BeyondHome) was established as a 501(c)3 nonprofit organization in 1987 with a mission to provide housing and opportunities for working families experiencing homelessness. BeyondHomes service enriches housing and focuses on self-sufficiency and families moving on the economic ladder. BeyondHome’s mission is to assist working families in their quest to go from homelessness to self-sufficiency for life. BeyondHome is a faith-based organization.
Job Description:
The Coach – Level 2 (C2) provides client-centered care that is timely, effective, efficient, and equitable. This candidate will be a compassionate, care-driven professional who believes in facilitating whatever is in the best interest of BeyondHome families, to help the family’s ability to become self-sufficient for life. The C2 takes the initiative in recommending and instituting BeyondHome coaching policies and procedures in conjunction with the Program Director.
Expected Availability:
Hours:
  • Full Time, 40 hours per week;
  • The exact hours will be determined by the needs of the assigned families.
Working outside of normal business hours expectations include:
  • Tuesday evening Group Nights 16 Tuesdays/year;
  • Special events: Easter Party, Summer Kickoff, Fall Festival, and Christmas Party;
  • Crisis intervention is necessary on evenings, nights, and weekends.
Remote and Hybrid Work:
  • Having face-to-face connections with BeyondHome families both formally and informally is crucial to family success;
  • One day/week can be done virtually;
  • That one day/week will be scheduled in conjunction with other program staff.
Responsibilities:
Assessment
  • Review client applications assessing the applicant’s level of need and potential for success in the BeyondHome Self-Sufficiency Program;
  • Interview potential clients when an opening at BeyondHome exists and determine any problematic issues that would be potentially harmful to the BeyondHome community, i.e., addiction, gang affiliation, mental health issues that are beyond the purview of BeyondHome resources, etc;
  • Meet regularly with assigned BeyondHome families to determine the underlying issues that are causing problematic behaviors or patterns which have or will inhibit that family’s progress toward self-sufficiency;
  • Work with each assigned family to jointly determine the current level of functioning in the categories defined by the Self-Sufficiency Matrix (SSM) quarterly;
  • Considering each family’s score on the SSM, together create a case plan that is updated quarterly to define the family’s goals and pathway to meeting those goals for that defined time.
Connection
  • Work to build a strong bond of connection with each family assigned to your caseload;
  • Determine issues that are the core causes of homelessness/poverty. Connect with and assist families in creating a plan for success and connect families to needed services and hold them accountable for working on their plans;
  • Use your connection to encourage and embolden each BeyondHome family to be vulnerable and transparent while progressing toward self-sufficiency;
  • Connect regularly with BeyondHome partners who provide services for families on your caseload to help determine progress and to troubleshoot when progress has stalled;
  • Supervise one BeyondHome group night class per trimester, building a connection with the facilitator and assisting the facilitator in any way needed.
Administration
  • Assess the level of functioning on each aspect of the Self-Sufficiency Matrix within 30 days of the family’s admission to the BeyondHome program and enter those scores in Salesforce. (Coach software);
  • Establish and enter an initial case plan in Salesforce with the family that outlines the initial steps they will take toward self-sufficiency and the timeline for each step within 30 days of the family’s admission to the BeyondHome Program;
  • Every quarter after admission, re-evaluate the family’s progress on the Self-Sufficiency Matrix and update the client’s Case Plan;
  • Gather, track, and document all required information in Salesforce, with every family encounter, for the joint purpose of tracking the progress of each family and having all information needed for grant reporting;
  • Take immediate, appropriate action in any crisis with resident safety being a top priority. The C2 will inform the Program Director immediately of any crisis. Formal documentation of said crisis will be completed by the C2 within 24 hours.
General Duties
  • Serve as a relational example to BeyondHome families in word and deed, even when not on duty;
  • Recognize that C2 is always representing BeyondHome. In so doing, C2 will maintain appropriate boundaries and will always be supportive and protective of BeyondHome in both word and deed, addressing any issues with Beyond Home among the staff and/or board of BeyondHome;
  • Commit to working out any conflicts with families, staff, or volunteers directly, only involving the people to whom the conflict pertains to maintain and demonstrate a high level of relational/emotional maturity, i.e., avoid triangulation or coalition building;
  • Maintain confidentiality of BeyondHome families, staff, board, and volunteers;
  • Meet regularly with the Program Director as agreed upon, recognizing that some periods will demand more frequent meetings than other periods;
  • Attend and actively participate in weekly all-staff and department meetings.

Qualifications

  • Commitment to the core values of BeyondHome as a Christian-faith-based organization;
  • Bachelor’s degree in social work or related field (master’s degree preferred) or at least two years of experience in a similar setting;
  • Commitment to supporting a culture of continuous improvement;
  • A creative mind with the ability to suggest improvements;
  • Ability to handle multiple concurrent assignments and meet deadlines as assigned.
