We are seeking a dedicated and compassionate Case Manager to join our team with Better is Better Home Care. As a Case Manager, you will be responsible for providing support and guidance to individuals in need, helping them navigate through various challenges and access necessary resources. Responsibilities:
- Conduct comprehensive assessments of clients' needs and develop individualized care plans
- Provide support services to clients
- Collaborate with other professionals and agencies involved in the client's care, including social workers, healthcare providers, and legal professionals
- Monitor clients' progress towards their goals and make appropriate adjustments to the care plan as needed
- Advocate for clients' rights and ensure they receive the necessary support and services
- Provide supervision and guidance to clients in developing life skills and achieving self-sufficiency goals

Requirements:
- Proven experience in case management or related field
- Knowledge of social work principles, motivational interviewing techniques
- Strong organizational skills with the ability to manage multiple cases simultaneously
- Excellent communication and interpersonal skills
- Ability to work effectively with diverse areas
- Proficiency in documentation and record keeping
