Better Home Health inc is Hiring a Family Model Provider (FMP) Near Nashville, TN

Job Overview:

Family model residential support is provided to adults who have been diagnosed with developmental disabilities. With this service, individuals are placed in homes with non-familial care providers who are well-trained in providing them with the assistance that they need. The main goal of this service is to promote learning and improvement of skills that will help them live independently and successfully in a family environment.

As a Family Model Provider (FMP), contracted families provide services in their home to adults that have an intellectual or developmental disability as a primary diagnosis. We match individuals with their families based on preferences, interests, routines, and support needs. This helps ensure a stimulating, engaged lifestyle for the individuals supported and fosters long-lasting stability in those support relationships.

Better Home Health, LLC is approved to provide Family Model services to both the DIDD Waiver and ECF/Choices Enrollees. Family Model Providers are needed to provide room and board in their own homes (apartment or house) to one or two people with disabilities. The Family Model Provider will be responsible for providing person-centered direct personal care including activities of daily living such as cooking, bathing, cleaning, medication administration assistance and medical appointments. Providing supports that will help the people supported reach their goals and outcomes as per the PCSP is paramount to the well-being, independence, and growth of the people supported.

Qualifications and Experience

1. Must have at least 1 year experience working with people with intellectual disabilities.

2. Must pass criminal background check

3. Must have an adequate and compliant owned or rented home or apartment with a dedicated bedroom for the people supported. Must be wheelchair accessible if the person supported uses a wheelchair.

4. Must have a reliable vehicle with auto insurance

5. Must complete all required training, including but not limited to RELIAS, Medication Certification, and CPR/FA.

6. Must have/obtain a computer with internet access for documentation purposes.

Fees or Payment for Services: We pay a competitive rate. Our Family Model Providers are contracted (1099) and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.

Application Process: Interested applicants must submit a resume to bettercare@better-homehealth.com. The potential FMP must attend an interview and then go through screening & background checks. We want to ensure that our persons supported with the right Family Model Provider. This process may take weeks or months.

About the FMP: Family Model Providers should reside in or around Davidson, Rutherford, Williamson, and Wilson counties. The FMP must provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. Reliable transportation and vehicle insurance is required as the Family Model Provider will need to transport the Member into the community. The Family Model Provider must be willing to follow through with any medical appointments. It is recommended that the Family Model Provider have a backup support system.
