Position Overview:

The coordinator will supervise staff of Bethany Village Home Care at Crispin Hill, assess resident conditions, and assist the running of Home Care Services at Appleridge.

Essential Duties & Responsibilities:

• Direct supervision of Home Health Aides/Personal Care Aides.
• Team leader who assist staff in planning and encourages independence while fostering of resident centered environment.
• Keep RN Coordinator informed daily of any concerns or problems in relations to the staff and residents.
• Effectively communicate with residents, staff, families, physicians, and outside agencies.
• Responsible for pre-pouring medications and ordering meds as needed.
• Reviews daily care sheets and PCC for all shifts to address any concerns or lack of documentation.
• Completes dr. appointment paperwork and follows up on orders. Schedules Dr. appointments as needed.
• Will assist with new admissions, entering in PCC as well as helping RN reviews meds and enter if needed.
• Assist Administrator in conducting audits as needed, as well as, participating in Quality meetings.
• Responsible for maintaining confidentiality of resident and facility information.
• Adheres to all fire safety & evacuation procedures.
• All other duties as assigned

Basic Duties & Responsibilities:

• Coordinate daily services among three shifts.
• On call responsibility
• Good judgment is required in order to make independent decisions.
• Administer injections and follow physician orders for all treatments and procedures.

_ Education Required: _

• Licensed Practical Nurse.
• Ability to read, write and speak English.

Experience Required:

• 3 to 5 years nursing experience.
• Strong communication and supervisory skills

Hazards:

• Exposure to the following may be present in this position:
• Blood-borne pathogens and infectious body fluids/waste.
• Needles.
• Combative residents.
• Disinfectants and cleaners.
• MSDS sheets are available for all chemicals used.

Working Conditions:

• Working conditions for this position may include the following: inside, odors, adequate lighting, working with/around others, working alone.

Physical Demands:

• Moderate lifting required at times:
• ~25 lbs, @ 5% of time.
• <25 lbs, throughout the day.
• Constant walking, standing, turning crouching, sitting, talking, hearing, seeing, and concentration are required for this position.

Personal Protective Equipment & Uniform Requirements:

• Uniform / Scrubs
• Appropriate Personal Protective Equipment when performing personal care, injections, dealing with blood or body spills, and working with chemicals (cleaning agents).
• Bethany Village Photo I.D. Badge.

Job Type: Full-time

Pay: $27.00 - $32.00 per hour

Expected hours: 37.5 per week
