Description

ORGANIZATION:

Bethany Community Services, Inc. is a non-profit organization providing housing and supportive services to seniors in Haverhill, MA and surrounding communities. Founded in 1965, Bethany manages 550 units of senior independent housing and oversees 33 certified Assisted Living units spread over five separate communities. Bethany is a mission-driven organization of approximately one hundred and forty full and part-time employees and is overseen by a volunteer Board of Directors.

SUMMARY:

The Resident Service Coordinator (RSC) plays an integral role as part of the Wellness team. In this position, the RSC will connect residents to supportive services provided by Bethany, public agencies, or private practitioners within the general community. The RSC is the primary point of contact for services which may include, but is not limited to, personal care, health care, wellness, meals, transportation, or adult day care. We are seeking a part-time 20 hour per week RSC for our new building in Haverhill. 

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Learn, understand, and participate in Bethany’s mission, vision, and values.
  • Provides general case management (including intake) and referral services to all residents needing such assistance.
  • May provide formal case management (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident’s situation and needs) for a resident when such service is not available through the general community.
  • Conduct initial assessments with new residents, identifying needs, interviewing residents and families, following organizational procedures to collect data, maintain privacy and confidentiality, and provide education, service and support to all residents.
  • Assess needs and help determine eligibility for services.
  • Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.
  • Make referrals internally or externally to appropriate agencies, as necessary.
  • Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual. Manages the provision of supportive services where appropriate.
  • Works closely with Supportive Services staff (Wellness Nurses and Certified Nursing Assistants), who provide care to the residents and perform wellness related services and activities.
  • Coordinates care with the resident, family member(s), physician(s), insurance companies, Area

Agency on Aging (AAA), Aging Service Access Point (ASAP) and other health care providers and relevant parties.

  • Establishes linkages with all agencies and service providers in the community, creates strong relationships with these agencies, including but not limited to, skilled nursing care centers, local hospitals, rehabilitation centers, home health agencies, home care agencies, and hospice providers.
  • Establishing a service plan for residents receiving care and coordinating internally with Wellness

Staff in the provision of care.

  • Attending marketing and community events to help promote Bethany’s programs and services, including housing and services.
  • Helps residents build informal support networks with other residents, family, and friends for increased socialization.
  • May educate other staff on the management team on issues related to aging in place and service coordination, to help them better work with and assist the residents.

Requirements

KNOWLEDGE/SKILLS/ABILITIES:

  • Knowledge of Federal and State programs (Medicare, Medicaid, MassHealth, Group Adult Foster

Care, PACE, Senior Care Options (SCO’s), Home and Community Based Waiver Program,

Massachusetts State Home Care Program).

  • Organizational Skills, Strong Written and Verbal Communication Skills, Interpersonal

Communication, and ability to work well as part of a multi-disciplinary team.

  • Ability to manage project guidelines, objectives, follow-through, and fulfillment.
  • Commitment to personal and professional development and continuing education.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • A bachelor’s degree in social work, Gerontology, Psychology, or Counseling is preferred.
  • Applicants without a degree as listed above must have appropriate work experience in resident service coordination or in a related field.
  • Supervisory or management experience is recommended but not required.
  • Training in the aging process, elder services, disability services, eligibility for and procedures of

Federal and applicable State entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol use and abuse, and mental health issues.

  • Two to three years’ experience in social service delivery with the elderly and nonelderly disabled is preferred.
  • Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled.
  • Demonstrated ability to advocate, organize, problem solve and provide results for the elderly and non-elderly disabled.

COMPENSATION & BENEFITS:

This position does have paid holidays and PTO. Employees in this role are eligible to enroll in our retirement plan through Nationwide.
