As a PARC Case Manager, you will be primarily responsible for providing post adoption services in the branch’s respective state and county. This individual is responsible for duties performed as part of the PARC team related to the three Services delivered in PARC: 1. Case management, Education and Support, and Training, Information Dissemination, Communication and Technology.

ESSENTIAL JOB RESPONSIBILITIES:

  • Contact families within 24 hours and complete face-to-face interviews within 48 hours of the referral;
  • Initiate information and referral services to all families within 4 business days of a request from a family or referral source
  • Follow referral and documentation procedures received from families and agencies;
  • Provide emergency intervention services over a 6 to 8-week period to assist families in crisis;
  • Develop a Family Centered Plan to help the family identify their strengths, weaknesses, and needs;
  • Provide information on available services through and in the community, which specifically address the family’s needs; obtain releases and authorizations as necessary;
  • Develop family progress reports to be completed on at least a monthly basis;
  • Utilize Specific Assistance; obtain Supervisor approval as required;
  • Ensure case management families have the 24 hour on-call phone number for use during non-traditional hours;
  • Maintain regular contact with the family, at least once per week, until case closure;
  • Meet with the family at case closure to evaluate service effectiveness and identify solutions to assist with unmet needs;
  • Complete a termination report, which the family signs, within 2 weeks of case closure which includes identifying information, reason for closure, record of contacts, service outcomes, and recommendations;
  • Provide one-on-one support, and liaison services for families in identifying and accessing community services;
  • Ensure families who adopt children who require specialized educational services are informed of Michigan laws, how to access the services, and provide support and advocacy in accessing these services as necessary;
  • Assist the Family Support Specialist in facilitation of monthly youth and parent support groups;
  • Assist the Program Supervisor by preparing documentation in a timely manner to complete and submit mandatory reporting;
  • Participate in weekly individual and monthly team supervision;
  • Complete all training requirements as a new hire or existing staff, including program specific topics;
  • Complete other duties as assigned.

QUALIFICATIONS:

  • Bachelor’s or Master’s level degree in Social Work, Human Services, or equivalent field of study from an accredited college;
  • At least two(2) years of post-education experience working in the field of child welfare, with adoptive and post-adoption experience preferred;
  • Active or eligible to obtain professional licensure in the state of the work location;
  • Demonstrated clinical and assessment skills;
  • Knowledge of normal developmental stages and the seven core issues that are specifically related to adoption and guardianship;
  • Demonstrated knowledge of state, community, and agency resources;
  • Ability to engage with, and relate to, families with multiple needs and challenges;
  • Ability to establish good relationships with networking capabilities with other agencies providing services to families in the communities served;
  • Willingness and ability to work flexible hours, sometimes after normal business hours;
  • Excellent verbal and written communication skills;
  • Computer and typing skills sufficient to perform essential job functions;
  • Active motor vehicle license and a reliable automobile with adequate insurance coverage;
  • Pass a criminal history screen, including state and local child protection agency registries;
