Best Point Education & Behavior Health is Hiring a Care Coordinator, Ohio RISE Near Cincinnati, OH

**$1000 Sign On Bonus_

Best Point is seeking an Ohio RISE Care Coordinator to provide family-centered and community-based wraparound services. Ohio RISE Care coordinators work with each family to develop a care plan, including transition plan and crisis plan. Develops and coordinates the wraparound team for each child served based on individual needs and strengths. Participates in after hours, on call response. Conducts regular assessment using the Child and Adolescent Needs and Strengths (CANS). Care coordinators demonstrate commitment to working with diverse populations.
Direct Client Service

  • Provide structured service planning and care coordination through high-fidelity wraparound.
  • Intensive Care Coordination (ICC) maximum caseload of 10; Moderate Care Coordination (MCC) maximum caseload of 25.
  • Initial face to face, community-based visit within 2 days (ICC) or 7 days (MCC) of referral.
  • Conduct comprehensive assessment using the Child and Adolescent Needs and Strengths tool (CANS) at required intervals.
  • Develop a crisis safety plan within 14 days of referral. Provides coordination and/or assistance in crisis management and stabilization as needed.
  • Develop and update the child and family-centered care plan within 30 days and at specified intervals thereafter.
  • Ensure services are delivered in accordance with the care plan.

Internal and External Collaboration*

  • Convenes and facilities child and family team meetings within 30 days and at specified intervals thereafter.
  • Assists in gaining access to essential community resources, linking to formal community services/systems. Provides assistance to clients in accessing natural supports within the community.
  • Develops and maintains a working knowledge of community resources maintaining updated information regarding juvenile justice, human services, health, mental health, legal, and educational/vocational systems of care.
  • Assists the transition of children through systems of care, including inpatient or residential, ensuring continuity and collaboration with caregivers and treatment team.
  • Attends program or community meetings as required.

Administration*

  • Responsible for providing a team meeting agenda, taking notes from the meeting, and delivering the documentation to each participant.
  • Adheres to all agency and regulatory standards and expectations. Responsible for meeting administrative expectations including, but not limited to maintaining required documentation and reporting data.
  • Ensures all required documentation is submitted accurately and on a timely basis.

Quality Assurance and Effectiveness*

  • Obtain and maintain certification for Child and Adolescent Needs and Strengths (CANS) assessment.
  • Trained in the moderate or intensive (as assigned) care coordination model with Ohio Center of Excellence.
  • Participate in continuous quality improvement peer review process.

Fiscal Responsibility*

  • Maintains active caseload at specified volume.
  • Provides and documents all required services within the timeframes.

Minimum Education, Experience and Other Skill Requirements:*
This position requires a Bachelor’s or Master’s degree in Social Work, Counseling or related field from an accredited college or university plus a minimum of three years experience in working with children and families.
Have a background in family systems, community systems, case management, child and family counseling, child protection or child development. Have qualifications and experience to work with children and families who are experiences serious emotional disturbance, trauma history, co-occurring mental health and substance use disorders and/or those involved with multiple systems of care.
Must be culturally responsive with training and experience to manage complex cases.
Must possess minimum technical skills including familiarity with Microsoft Windows and ability to use standard Microsoft Office applications
