- Responsible for treatment planning and care coordination
- Take care of insurance verification and issue resolution
- Review and attachment of documents for claim submission
- Handle updating of forms for compliance, i.e., translation updates, patient education, etc.
- Manage and track patient referrals
- Facilitate urgent referral for patients
- Direct patients to appropriate specialist referral
- Assist in obtaining consultation/specialists reports back to the health center
- Ensure that the patients/families have adequate understanding of referral procedures and the reason for specialist referrals
- Provide education and support to patients/families about the importance of keeping the specialist referral appointments
- Assist staff with orientation and training of volunteers and trainees
- Assist patients with scheduling appointments with specialists
- Collaborate with social workers to provide assistance and support to patient with financial or health insurance issues
- Act as the point of contact for patients/families with issues related to referrals
- Generate weekly referral and test tracking EDR reports
- Follow up with patients post-specialist visits
- Perform other duties as assigned
