Job Title: Long Term Care (LTC) LPNJob Summary:As a Long Term Care (LTC) LPN, you will provide comprehensive nursing care to patients residing in long-term care facilities. You will work closely with other healthcare professionals to ensure the delivery of quality care and to promote the wellbeing of residents. Your primary responsibilities will include assessing patients, creating care plans, administering medications, monitoring vital signs, and assisting in daily activities.Responsibilities and Duties:1. Perform initial and ongoing assessments of residents' health conditions.2. Develop individualized care plans in collaboration with the healthcare team.3. Administer medications and treatments as prescribed by physicians.4. Monitor and document residents' vital signs and overall health status.5. Assist residents with activities of daily living, including bathing, dressing, and feeding.6. Collaborate with other healthcare professionals to coordinate specialized care services.7. Ensure compliance with healthcare regulations, facility policies, and infection control protocols.8. Maintain accurate and up-to-date residents' medical records and document provided care.9. Educate residents and their families on disease management, medications, and self-care techniques.10. Support and promote residents' physical and emotional wellbeing.11. Respond to medical emergencies and provide immediate assistance.12. Collaborate with social workers and case managers to address residents' social and emotional needs.13. Observe and report any changes in residents' condition to physicians and supervisors.14. Participate in interdisciplinary care conferences and provide input on care plans.15. Continuously update knowledge and skills through ongoing education and professional development.Qualifications and Skills:1. Valid LPN license in the state of [state].2. Graduation from an accredited practical nursing program.3. Proven experience in a long-term care setting.4. In-depth knowledge of nursing principles and practices in long-term care.5. Strong understanding of geriatric care and the unique needs of elderly patients.6. Excellent communication and interpersonal skills.7. Ability to multitask and prioritize tasks effectively.8. Empathy and compassion towards the elderly population.9. Strong critical thinking and problem-solving abilities.10. Ability to work independently and as part of a team.11. Proficiency in medical record-keeping and documentation.12. Knowledge of infection control procedures and safety protocols.13. Basic computer skills and ability to use electronic medical records systems.14. Availability to work flexible hours, including weekends and holidays.15.
