Job Description

Job Title: Behavioral Health Care Manager

Job Summary:

The Behavioral Health Care Manager will be responsible for coordinating and providing comprehensive care management services to individuals with behavioral health needs. This position will involve assessing clients, developing care plans, coordinating with healthcare providers, and monitoring treatment progress to ensure positive outcomes.

Responsibilities and Duties:

  • Conduct assessments and develop individualized care plans for clients with behavioral health needs
  • Coordinate care with other healthcare providers, including psychiatrists, therapists, and social workers
  • Monitor treatment progress and adjust care plans as needed to ensure positive outcomes
  • Provide individual counseling and support to clients as needed
  • Advocate for clients and connect them with community resources to support their mental health and well-being
  • Maintain accurate and up-to-date documentation of client interactions and treatment plans

Qualifications and Skills:

  • Bachelor's degree in social work, psychology, or a related field
  • Licensure as a social worker or counselor preferred
  • Knowledge of behavioral health diagnoses and treatment options
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced, team-oriented environment
  • Experience with care coordination and case management preferred
