Position SummaryProvide administrative, clerical, and customer service support to the various units of the Human Services and/or Public Health Departments.
EXAMPLE OF DUTIES
Duties and ResponsibilitiesEssential FunctionsA. Provides customer service, in-person, by phone, and by e-mail.B. Coordinates or explains placement, financial process, policies and procedures to clients and others.C. Completes all necessary department paperwork.D. Maintains documentation and scanning.E. Determines eligibility of clients for social services or public health programs.F. Explains social service, income maintenance and other community/public health programs and services to individuals or groups.G. Interviews clients to obtain and verify necessary factual information.H. Coordinates and facilitates programs.I. Coordinates and implements programs.J. Performs data entry into multiple County and state computer systems and retrieves information from those systems.K. Responds to initial complaints of clients.L. Prepare periodic reports as required.M. Manage inventory of non-office supplies.N. Maintains knowledge and training in program requirements and local, state, and federal computer systems and programs.O. Participates in inter-disciplinary team meetings and staff meetings. Additional FunctionsA. Attends trainings as needed.B. Performs other duties as assigned or apparent.CASE AIDE PUBLIC HEALTH DEPARTMENT - ADDENDUMSCASE AIDE-PUBLIC HEALTH SUPPORTA. Perform outreach activities and assist in implementation of the Child and Teen Checkups program. A.1. Create educational letters, mailings, and text alerts. A.2. Provide local resources for clients to raise awareness of the program and ensure children are receiving regular wellness checkups. A.3. Coordinate with other staff to work on development and evaluation of program. A.4. Create reports. A.5. Implement new outreach strategies to improve the program. A.6. Conduct phone calls to clients. A.7. Complete other implementation activities as assigned.B. Implement outreach activities and assist in the implementation of the Follow Along Program. B.1. Create educational letters, conduct mailings, print screening due notices, and screen questionnaires. B.2. Refer parents of children with suspected delays to Public Health Nurse for further evaluation and follow-up and provide additional resources and developmental activities. B.3. Create reports and coordinate with other staff to work on development and evaluation of program.C. Process Public Health referrals including but not limited to Family Home Visting (postpartum, antepartum, Health plans), WIC, breastfeeding, and Disease Prevention and Control. C.1. Maintain sensitive information. C.2. Create client charts and files within PH-Doc and program documentation system. C.3. Create educational packets provided to clients. C.4. Facilitate document attachments and communication of information to Public Health Department staff is included within referral intake.D. Conduct office support duties for the Public Health Department including but not limited to monitoring and responding to department phone calls, department emails, department mail.E. Monitor department supply inventories, restocking of supplies, and facilitation of purchase order request processes for the Department.F. Provide training and guidance on administrative duties listed in job description to newly hired staff.G. Serve as backup to fellow Public Health Department Case Aide and other Department staff, as assigned. H. Assist and coordinate with Public Health Department staff on preparation and development of statistical reports, workplans, program evaluations and outreach activities. I. Assis in Public Health Emergency Response activities. J. Other duties as assigned.CASE AIDE PUBLIC HEALTHA. Support of the WIC program. A.1. Receive and process all WIC intake/referrals per phone, fax, mail or online. A.1.1. Review for eligibility and schedule via the WIC software program. A.1.2. Re-evaluate WIC eligibility per Separation of Duties requirement. A.2. Responsible as Local System Administrator for all WIC computer hardware and software, performing hardware set-ups, network-trouble shooting, database maintenance, state imports and exports of data, and computer support and mentoring for staff. A.3. Monitor bio-hazardous waste. A.4. Act as back-up for WIC lab services, including weight, height, Hgb and lead.B. Assist in facilitating the Child Passenger Safety program. Must acquire Child Passenger Safety certification within six months of hire. B.1. Perform car seat checks, car seat distribution, complete paperwork and maintain car seat inventory. B.2. Determine eligibility for free/reduced car seats.C. Assist with the implementation of the Child & Teen CheckUp Program.D. Work with the Human Services Fiscal unit in performing billing for the Public Health Department. D.1. Create monthly reports and conduct PH-Doc statistics from dailies reports. D.2. Identify coding/entry errors in PH-Doc and make corrections. D.3. Complete monthly Public Health billing.E. Serve as PH-Doc Superuser. E.1. Provide ongoing support to Public Health staff on PH-Doc and set-up new users. E.2. Attend MnCCC PH-Doc meetings, provide updates to the Department, and train Users. E.3. Provide ongoing technical assistance and answer questions.F. Conduct office support duties for the Public Health Department including but not limited to retention practices, monitoring, and responding to department phone calls, department emails, department mail.G. Monitor department supply inventories, restocking of supplies, and facilitation of purchase order request processes for the Department.H. Provide training and guidance on administrative duties listed in job description to newly hired staff.I. Serve as backup to fellow Public Health Department Case Aide and other Department staff, as assigned.J. Assist and coordinate with Public Health Department staff on preparation and development of statistical reports, workplans, program evaluations and outreach activities.K. Assis in Public Health Emergency Response activities.L. Other duties as assigned. CASE AIDE SOCIAL SERVICES - ADDENDUMSCASE AIDE - CHILDREN SERVICESA. Pay bills by creating voucher in SSIS. A.1. Create and complete vouchers with accuracy and signatures. A.2. Contact providers, hospitals, doctors, and offices for outstanding bills. A.3. Answer all calls and inquiries regarding payments. A.4. Track due dates.B. Begin social and medical history when children go into placement. B.1. Request court orders. B.2. Maintain social and medical history.C. Work directly with Social Workers and Families to complete ICWA notifications.D. Provide transportation for parents to and from court, transport children to medical appoints and school while in placement.E. Complete IPCP paperwork for both outgoing and incoming for out of state placements. E.1. Communicate with the Sate of MN and provide necessary information. E.2. Complete social and medical history.F. Contact transportation coordinators to set up school transportation.G. Complete relative search letters for children in foster care.H. Track MAPCY due dates and work with social workers on deadlines.I. ICWA- sends out letters to tribes to determine if the children are ICWA. Track this information, relay to social workers, scan to SSIS and the courts.J. Request medical cards and acquire birth certificates for children in placement.K. E-file court documents and forward mail to SW in regard to e-files.L. Assist in looking and contacting placements for children going into foster care.M. Send out MAPCY rating documents whenever a new MAPCYis completed to foster families and facilities.N. Monitor car seats, availability, and expiration dates. Purchase new car seats as needed.O. Request new vendors as needed and provide information to set up new vendors to accounting department.P. Oversee additional programs such as “Share the Spirit”.Q. Maintain OnBase-use system.R. Request specific orders from Courts to acquire medical information on children for social and medical histories.CASE AIDE - HOME & COMMUNITY BASE SERVICES (HCBS)A. Support Contracted CSG case load. A.1. Release information and the reassessment work. A.2. Prepare letters and documents. A.3. Manage communication and data management for 60 client cases – supporting workflow to private agency and MNChoice Assessment staff for individuals not receiving county waiver. A.4. Case management includes file preparation.B. Transport clients from their location to court and other appointments. B.1. Maintain client information.C. Prepare new client files. C.1. Manage paperwork. C.2. Work with the Adult Services unit to prepare the OnBase files.D. Assist with Foster Care placement.E. Maintain service agreements, bills and vouchers for Case Manager clients and court non-client individuals.F. Create and manage resource logs.G. Assist with case coordination for PCA and CSG cases.H. Serve as training coordinator.I. Responsible to organize, coordinate and execute unit workflow between MNChoice/Adult Protection/Intake/Assessment Unit and Home and Community Based Services Unit. I.1. Maintain Assessment/Reassessment tracking, assignment logs, and Data Flow for Home and Community Based Services Unit of 750 clients cases requiring annual assessments. I.2. This requires knowledge of and access to Revation Communication System, Mn-ITS State System, SSIS, MMIS, MAXIS, Excel and OnBase to complete functions of the position and assure workflow across teams. I.3. Complete “paperless” data exchange. I.4. Set up cases for assessment and assure data transfer through to case management using SSIS and DataBank System. I.5. Complete all billable work requested by the case manager.J. Verify data entry, correct billing codes and work to make any necessary changes.CASE AIDE - MENTAL HEALTHA. Provide technical assistance to SSIS users. A.1. Troubleshoot issues. A.2. Contact helpdesk, if necessary. A.3. Verify reports and information in SSIS. A.4. Schedule SSIS team user meetings.B. Designated SSIS Mentor. B.1. Provide ongoing support to social services staff in SSIS. B.2. Set-up new users. B.3. Train users. B.4. Provide ongoing technical assistance and answer questions. B.5. Set users up to scan to their e-mail from two copiers.C. Meet with parents of kids in placement to gather IVE information. C.1. Verify data. C.2. Discuss potential fees parents may occur, while children are in placement. C.3. Verify wage stub data, bank statements, child support, adoption assistance, social security, and other information that includes financial and public health.D. Manage and update county website for Human Services.E. Verify data entry, correct billing codes and work to make any necessary changes.F. Conduct off year daycare visits using ELICI checklist checking for capacity, fire extinguisher, working smoke detectors, hot water temperature, cribs, training, and answer daycare provider questions. F.1. Review and process off year daycare visit paperwork. F.2. Ensure training is up to date.G. Address IT issues like change printer cartridges, clear jams, set up webinars, web ex and ITVs.H. Work with MNIT with issues regarding ITV equipment.I. Clear clients in SSI and SMI.J. Clean up errors in the system. J.1. Check metafile to delete each month for Social Services and Public Health. J.2. Search metafile for documents needed by social workers.CASE AIDE - SOCIAL SERVICES SUPPORTA. Assist with open case files. A.1. Verify correct information in SSIS. A.2. Open eligibilities. A.3. Create service plans.B. Create mandated report letters. B.1. Mail one copy to the mandated reporter. B.2. Upload copy into SSIS.C. Schedule ITV conference calls with DHS and setup equipment.D. Convert and transfer Child Protection audio files and transcribe files when requested.E. Request and follow up on medical and school records for children in placement.F. Manage all incoming social service mail. F.1. Sort mail. F.2. Determine correct workgroup information for each document by searching client in SSIS. F.3. Import and index mail documents into OnBase to route to appropriate worker’s workflow. F.4. Stamp imported mail and put in appropriate worker’s mailbox, route any St. Cloud office worker mail.G. Manage all incoming Social Services faxes. G.1. Determine correct workgroup information for each document by searching client in SSIS. G.2. Import and index mail documents into OnBase to route to appropriate worker’s workflow.H. Assist with voucher payment process. H.1. Import all client bills into OnBase and index to the appropriate worker’s workflow. H.2. Once bill is signed by worker and supervisor, print the bill. H.3. Ensure all information is correct as listed on the voucher based on the received client bill. H.4. Attach signed bill to yellow voucher and route to accounting.I. Enter Mental Health diagnosis codes, updating supplemental eligibility and current information in SSIS.J. Manage invalid information and hold in workgroup folders.K. Closed file scanning for electronic files. K.1. Generate report to determine closed cases. K.2. Create an electronic closed file form and save to the electronic file in OnBase. K.3. Close eligibilities in SSIS. K.4. Note any closed files containing a school interview by flagging application with “SS Interview” note for future destruction process.L. Type performance reviews, letters, and appeals to departments and judges. L.1. Assemble documents for appeals. L.2. Manage filling of documents and records.M. Type Home Studies for Child Foster Care Unit.N. Redact documents for record and court requests.O. Close case files. O.1. Close eligibility in SSIS. O.2. Prep and scan file.P. Mail Adult Foster Care trainings.Q. Transcribe meeting notes for staff and Labor Management Committee.R. Complete background checks as needed and requested by Child Protection staff.S. Assis with off year daycare relicensing home visits.T. SSTS instructor/validator.U. SSIS mentor.
