Registered Nurse Case Manager
Overview:
We are seeking a compassionate and experienced Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will play a vital role in providing patient care and coordinating services for individuals in need. This is an opportunity to make a difference in the lives of patients and their families by ensuring they receive the highest level of care and support.
Responsibilities:
- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Coordinate and manage patient care across various healthcare settings
- Collaborate with physicians and other healthcare professionals to ensure comprehensive and effective treatment plans
- Monitor patient progress, evaluate outcomes, and make necessary adjustments to the care plan as needed
- Provide education and support to patients and their families regarding their healthcare needs, treatment options, and available resources
- Utilize EMR software to document patient information, track progress, and communicate with the healthcare team
Requirements:
- Valid Registered Nurse (RN) license
- Strong knowledge of patient assessment techniques
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Proficient in using electronic medical records (EMR) system
