Overview:
We are seeking a compassionate and organized Bilingual Home Care Coordinator to join our team. The Home Care Coordinator will play a vital role in assisting clients and their families in navigating the complexities of home care services. The ideal candidate will possess excellent communication skills in both English and Spanish, along with a strong understanding of healthcare systems and community resources.

Responsibilities:

  • Client Assessment: Conduct comprehensive assessments of clients' needs and develop personalized care plans in collaboration with clients, their families, and healthcare professionals.
  • Coordination: Coordinate home care services, including scheduling caregivers, arranging transportation, and managing medical appointments.
  • Communication: Serve as a liaison between clients, caregivers, healthcare providers, and community resources to ensure continuity of care.
  • Advocacy: Advocate for clients' rights and access to appropriate services, addressing any barriers or challenges they may encounter.
  • Education: Provide clients and their families with information on available resources, disease management, and self-care techniques.
  • Documentation: Maintain accurate and up-to-date records of client assessments, care plans, and service delivery in compliance with regulatory standards.
  • Crisis Intervention: Respond to emergencies and crisis situations promptly, providing support and guidance to clients and their families.
  • Quality Assurance: Monitor the quality of care provided to clients, conducting regular evaluations and implementing improvements as needed.
  • Training: Provide training and support to caregivers to ensure they have the necessary skills and knowledge to deliver high-quality care.
  • Cultural Competence: Demonstrate sensitivity to cultural differences and respect for clients' diverse backgrounds and beliefs.

Qualifications:

  • Bachelor's degree in Social Work, Nursing, Psychology, or related field (preferred).
  • Previous experience in home care coordination, case management, or a related role.
  • Fluency in English and Spanish.
  • Strong communication, interpersonal, and organizational skills.
  • Knowledge of healthcare systems, community resources, and government programs.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate, empathetic, and nonjudgmental approach to client care.
  • Proficiency in computer skills and electronic health record systems.
