General Job Description: Responsible for service coordination and outreach services to patients • Provide intensive home visitation services for pregnant and parenting families • Promote the health and well-being of clients by informing potential consumers about available services • Provide transportation services to patients of Ben Archer Health Centers when appropriate and available • High School graduate or GED is required; college degree in Social Work, Early Childhood Development, Marriage and Family Counseling, Infant Family Studies or a related field is preferred • Two years community outreach experience in a social services function and experience with the early childhood population is preferred • Must have current NM Driver’s License and a clean driving record • Must pass a background check • Ability to speak English and Spanish is preferred • Maintains appropriate confidentiality • Must possess and maintain a clean driving record

Job Type: Full-time

Pay: From $13.00 per hour
