Immediate Opening: Social Worker/Care Coordinator – Full-Time, First Shift

Beloit Health System has an immediate Care Coordinator opening. This position offers a competitive wage, with generous benefits, all while working for a Health System with over 50 years of experience that is known for our friendly atmosphere and focus on patients.

This position will:

• Review new admissions for any special needs or issues that need to be addressed
• Assist with discharge planning and coordination of transition of care
• Provide and help patients understand financial, emotional and social resources available to them
• May work occasional Saturday hours

Required:

• Bachelor’s Degree in Social Work
• Wisconsin Social Work license
• Medical social work experience preferred, but not required
