We are actively seeking a Registered Nurse Case Manager to join our team covering the High Desert areas.

RNCMs enjoy a hybrid role of hands-on 1:1 patient care, and administrative case management.

• We have imposed limits on daily and weekly visits, and allot daily/weekly charting time to aid in keeping accurate charts and preventing nurse burnout

• We have competitive pay. Base Full time salary range (including benefits) $90,000-$100,000

• We have zero on-call or after-hour requirements with full time employment, but have plenty of opportunities for additional compensation (after hours and weekend visits) if desired

• We have multiple opportunities for career advancement and progression

• Hospice experience is preferred

Responsibilities including but not limited to:

• Establishing contact and maintaining rapport with patient/family/caregivers

• Visiting patients in accordance with plan of care, assessing for needs (DME, Medication, Change in Condition etc), and completing all necessary documentation as required by Medicare COPs

• Maintain communication and report ongoing condition of patients to office and supervisory staff, and complete IDG notes, POCs, CTIs etc., in accordance with Medicare COPs

• Current Professional RN License
• Valid Drivers License
• Current Auto Insurance
• Physical within last year
• TB test within last year
• Hospice Experience is preferred
