We are actively seeking a Registered Nurse Case Manager to join our team covering the Inland Empire and High Desert areas.
RNCMs enjoy a hybrid role of hands-on 1:1 patient care, and administrative case management.

-We have imposed limits on daily and weekly visits, and allot daily/weekly charting time to aid in keeping accurate charts and preventing nurse burnout
-We have competitive pay. Base Full time salary range (including benefits) $90,000-$105,000
-We have multiple opportunities for career advancement and progression
-Hospice experience is preferred
Responsibilities including but not limited to:
-Establishing contact and maintaining rapport with patient/family/caregivers
-Visiting patients in accordance with plan of care, assessing for needs (DME, Medication, Change in Condition, etc.), and completing all necessary documentation as required by Medicare COPs
-Maintain communication and report ongoing condition of patients to office and supervisory staff, and complete IDG notes, POCs, CTIs etc., in accordance with Medicare COPs
-On-call or after-hour requirements with full time employment
