We are seeking a skilled MDS Coordinator to join our healthcare team. The MDS Coordinator will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments for our patients. **Duties:**

- Conduct comprehensive assessments of patients' medical, functional, and psychosocial needs

- Ensure compliance with Medicare and Medicaid regulations

- Collaborate with healthcare professionals to develop individualized care plans

- Maintain accurate and up-to-date patient records

- Monitor and evaluate patient care outcomes

- Implement strategies to improve quality measures such as HEDIS

- Utilize ICD-10 coding standards for accurate documentation

- Adhere to HIPAA guidelines for patient confidentiality

**Requirements:**

- Valid Missouri nursing license

- Proficiency in Medicare regulations and reimbursement processes

- Knowledge of patient care best practices and standards

- Familiarity with coding procedures and documentation requirements

- Strong attention to detail and analytical skills
