About the Position
The Case Manager functions as part of a collaborative, interdisciplinary team responsible for providing supportive services to clients enrolled in Navos’ Standard Supportive Housing (SSH) program at Midway and the Intensive Community Support and Recovery Program (ICSRP) following established treatment guidelines, and agency policies and procedures.
  • Midway SSH is located in Kent, WA, and has up to 18 enrolled clients who live across 6 houses within a cul-de-sac with a staff office located in a shared 7th building. Up to 4 additional clients live together in a house nearby. Primary supports at Midway include assisting individuals with independent living skills, such as maintaining a shared household, grocery shopping, money management, medication management, crisis management, attending appointments, etc. The goal of Midway is to increase independent living and self-management skills.
  • ICSRP has up to 38 enrolled clients who live in and around Burien, WA, including in Navos-owned/managed housing. Primary services of this position include pre-engagement work, transition planning, linkages to needed services, case management, care coordination, discharge planning, and community and residential supports that promote recovery and successful community integration.
Position Function & Responsibilities
– Provides person-centered skills training, advocacy and case management for clients
– Assists in developing treatment plans which clearly identify the client’s voice, treatment strategies, and specific outcome measures
– Meets agency documentation standards
– Meets authorization requirements of contracts
– Strategically tailors effective clinical/case management services to meeting diverse client needs.
– Adheres to MHS and Navos Attendance and Punctuality Policy and Procedure standards, and maintains reliable attendance
– Contributes to the success of the organization by meeting organizational competency expectations and core values, continuously learning, and by performing other duties as needed or assigned

 Ability to utilize de-escalation techniques to assist clients with behavioral challenges
 Ability to respond to clients with empathy, maintaining their dignity and confidentiality
 Ability to interact with a wide variety of people, communicates effectively verbally and in writing, possesses strong interpersonal skills, and demonstrates kind and collaborative behavior
 Ability to work within the structure of a multidisciplinary team
 Knowledge of and skill in the use of personal computer and related systems and software

Education and/or Experience
Bachelor’s Degree in Social Work, Counseling, Psychology, or any other human services field required. Two (2) years of Behavioral Health experience may be considered in lieu of a Bachelor’s Degree
One (1) year of direct client care experience preferred
Must obtain Washington State Agency Affiliated Counselor registration within 60 days of hire unless a higher credential is held
Current Washington State Driver’s License and proof of a safe driving record (which meets established MHS and Navos Standards) obtained by a motor vehicle report from the appropriate state

The work may require the exertion of up to 50 pounds and may consistently require lifting, carrying, pushing, pulling or otherwise moving objects
May have infrequent exposure to various conditions while in the client’s environment
