Case Manager

Location: Glen Allen, VA, 23059

Job Summary

The Case Manager will be responsible for providing case management services to clients in need of assistance. This includes assessing client needs, developing and implementing care plans, coordinating services, and monitoring progress. The Case Manager will work closely with clients, families, and other service providers to ensure that clients receive the best possible care.

Key Responsibilities

  • Conduct initial assessments of client needs and develop care plans
  • Coordinate services and resources to meet client needs
  • Monitor client progress and adjust care plans as needed
  • Provide ongoing support and guidance to clients and families
  • Collaborate with other service providers to ensure comprehensive care
  • Maintain accurate and up-to-date client records
  • Participate in team meetings and case conferences
  • Adhere to all ethical and professional standards

Qualifications

  • Bachelor's degree in social work, psychology, or related field
  • Minimum of 2 years of experience in case management or related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Excellent organizational and time management skills
  • Knowledge of community resources and services
  • Ability to maintain confidentiality and adhere to ethical standards

Physical Demands

The Case Manager may be required to sit or stand for extended periods of time and may need to lift up to 25 pounds. The position may also require driving to client homes or other locations.

Work Environment

The Case Manager will primarily work in an office setting but may also need to travel to client homes or other locations. The position may require some evening or weekend work.
